The investigation report confirmed that the causes of the accident were serious dereliction of duty by the site management, lax review of key information such as contractor qualification and personnel qualification, the approval process was in name only, and the subcontracting of contractors layer by layer was ignored; Fail to carry out effective safety management on the Contractor's site operation, fail to review the Contractor's construction scheme and construction safety measures according to the safety regulations and put forward review opinions, fail to timely discover and stop the illegal warehouse clearance operation, fail to implement the dangerous operation approval system, fail to manage the outsourcing construction team, and fail to implement the approval and signature system of the Confined Space Operation Permit according to the regulations. Failure to set up a leading group for inventory clearance in accordance with the requirements of the regulations, failure to formulate a plan for inventory clearance and on-site disposal, which were approved by the safety production department of the enterprise and the person in charge of the enterprise, and failure to exercise unified command over the inventory clearance operation. The safety management of confined space is not in place, and the safety precautions for confined space operation are not formulated before the implementation of confined space operation.
It can be said that the cement enterprise turned a deaf ear to the hidden dangers of major accidents and did not seriously eliminate them, which eventually led to tragedy. The lesson of blood is extremely profound.
