In accordance with the provisions of the Safety Production Law of the People's Republic of China and the Regulations on the Reporting and Investigation of Production Safety Accidents (Decree No.493 of the State Council), the People's Government of Zhongning County was established on October 16, 2024, headed by the Director of the County Security Committee Office and the Director of the Emergency Management Bureau. County Discipline Commission, County Public Security Bureau, Ning'an Town People's Government, County Emergency Management Bureau, County Industry and Information Bureau, County Federation of Trade Unions and safety production experts participated in the "10.11" general mechanical injury accident investigation team of Ningxia Yinghai Tianxiang Building Materials Co., Ltd. In accordance with the principles of "four not letting go" and "scientific and rigorous, in accordance with the law and regulations, seeking truth from facts and paying attention to practical results", the accident investigation team, through on-site investigation, technical demonstration, comprehensive analysis, access to information and inquiry of the parties concerned, ascertained the course, causes, emergency disposal, casualties and direct economic losses of the accident, and identified the accident. I. Basic information
(I) Basic information of the accident enterprise.
Ningxia Yinghai Tianxiang Building Materials Co., Ltd. was established on May 13, 2008, and is located in Ning'an Town, Zhongning County, Ningxia. Its legal representative is Guo Jun; its registered capital is RMB 200 million; its social credit code is 9164052167040678XM; It is mainly engaged in cement production, cement products manufacturing, brick and tile manufacturing, building stone processing, etc.
(2) Basic information of the accident device. The site of
the accident is the south end of the 1 # batching conveyor belt in the batching station of the grinding workshop. The whole conveyor belt is 118.8 meters long and 0.8 meters wide. The running speed is 1.
II. The course of the accident and the emergency disposal
(I) The course of the accident. At about 12:40 on October 11,
2024, Li Mooming, the on-duty monitor of the batching station in the grinding workshop of Ningxia Yinghai Tianxiang Building Materials Co., Ltd., found that the running 1 # batching conveyor belt deviated during the inspection. Without reporting to the person in charge of the workshop, the operator of the cement mill in the central control room, Mao Mou, was informed to suspend the operation of the 1 # batching conveyor belt without authorization, and to deal with the deviation of the belt. About 4 minutes later, in order to check the disposal of the belt, Li Mooming again informed Mao Mou to start and stop the 1 # batching conveyor belt at an interval of 4-5 seconds. About 30 seconds later, Mao Mou started and stopped the 1 # ingredient conveyor belt again under the notice of Li Mooming, and the start-stop interval was 4-5 seconds. At about 12:46, Li Mooming illegally pushed his left foot on the edge of the conveyor belt that was not completely returned to the right position and then directed Mao to start the 1 # ingredient conveyor belt for the third time. He tried to reset the conveyor belt at the moment of starting through lateral force. After receiving the starting instructions, Mao Mou cooperated with Li Mooming to start the 1 # ingredient conveyor belt in violation of regulations without consulting the relevant leaders or verifying the starting conditions, which caused Li Mooming to lose his balance at the moment of starting the conveyor belt and fall to the ground against the passive roller shield driven by the belt.
(2) Emergency disposal of accidents. After the
accident, Luo Mojun, a batching worker, found that Li Mooming was bleeding from his mouth and nose and lying on his back in the corridor on the west side of the 1 # batching conveyor belt in the process of checking the material level of the transfer station. He immediately used the walkie-talkie to call Huang Mou, who was in the mill, and Zhao Moyong, who was repairing the 3 # cement mill. After contacting them, he dialed his mobile phone to report to Su Moujiang, Minister of Production and Director of Grinding Workshop. Su Moujiang and Yue Moujie, vice president of process quality of the company, then arrived at the scene and found that Li Mouming's carotid artery had no fluctuation. Su Moujiang, together with Luo Moujun, Huang Mou and Zhao Mouyong, carried him outdoors. During this period, Cao Mou, Minister of Safety and Environment of the company, dialed 120 emergency calls after receiving Yue Moujie's notice. 10, the ambulance arrived at the door of the workshop, and the healthcare workers took first aid measures to Li Mooming on the spot, and then declared his death after the rescue was ineffective.
3. Casualties and direct economic losses

(1) Strengthen the construction of safety essence and implement the main responsibility of enterprises. Ningxia Yinghai Tianxiang Building Materials Co., Ltd. should draw lessons from the accident and improve the post operation rules and inspection system in time. Strengthen safety education and training, improve the safety awareness of employees, when the equipment is abnormal and needs to be shut down, the central control operator should fully communicate and confirm with the on-site operators, and handle the power outage and power transmission confirmation procedures, strictly implement the quarantine measures of power outage, locking and tagging for equipment maintenance, and arrange on-site monitoring. In daily work, we should strengthen the publicity of safety knowledge before and after the shift, focus on high-risk operations, eliminate illegal command and operation, and vigorously improve the management of the operation site.
(2) Carry out risk identification in a down-to-earth manner and effectively eliminate hidden problems. Ningxia Yinghai Tianxiang Building Materials Co., Ltd. should carry out risk identification and hidden danger investigation and management from top to bottom, scientifically assess the risk level, clarify the risk control measures under normal, abnormal and emergency conditions, formulate and implement risk control measures according to the results of risk identification, enhance the ability of hidden danger investigation and management, and strengthen the safety of equipment and facilities. Improve the protective facilities for the rotating parts of mechanical equipment, restore and ensure the use function of the pull rope switch of the belt conveyor, and supplement and improve the protective facilities on both sides, head and tail of the belt conveyor to achieve intrinsic safety. Improve video surveillance, and set up constant safety lighting and emergency lighting facilities in areas with insufficient environmental lighting in production workplaces.

County emergency management departments should continue to conduct industry supervision pressure, concentrate on safety inspection of enterprises using the same process equipment, prevent and resolve hidden dangers of similar accidents, and further guide enterprises to improve their intrinsic safety production level. County industrial and information departments should continue to consolidate their daily supervision responsibilities, focusing on the supervision and inspection of enterprise safety education, equipment inspection and maintenance.
