In accordance with relevant laws and regulations such as the Production Safety Law of the People's Republic of China and the Regulations on the Reporting, Investigation and Handling of Production Safety Accidents (Decree No.493 of the State Council), the Qingtongxia Municipal People's Government has established the "7.20" General Collapse Accident Investigation Team of Ningxia Jinyuyuan Resources Recycling Co., Ltd. (Hereinafter referred to as the Accident Investigation Team) to carry out accident investigation according to law. In accordance with the principles of "four not letting go" and "scientific and rigorous, in accordance with the law and regulations, seeking truth from facts and paying attention to practical results", the accident investigation team, through on-site investigation, investigation and evidence collection, access to information and analysis and demonstration, found out the course and cause of the accident, identified the nature and responsibility of the accident, put forward suggestions and preventive measures for the accident, and formed a report on the investigation and handling of the accident. After collective discussion and deliberation by the members of the accident investigation team, they agreed to the suggestions of the investigation report on the analysis of the cause of the accident, the identification of the nature of the accident, the handling of the accident unit and the relevant responsible personnel and the preventive measures.
According to the Circular of the Office of the Security Committee of the State Council on the Issuance of Measures for Assessing the Implementation of Measures for the Prevention and Rectification of Production Safety Accidents (No.4, 2021) and the Measures for Assessing the Implementation Effect of Measures for the Rectification of Production Safety Accidents in Wuzhong City. 4. Xu Mou, the head of the raw material device of Ningxia Jinyuyuan Resources Recycling Co., Ltd., was fined forty thousand nine hundred and eighty-seven yuan (40987 yuan) in accordance with the provisions of Article 96 of the Safety Production Law of the People's Republic of China. 5. 4. Implementation of
rectification measures 1. Ningxia Jinyuyuan Resources Recycling Co., Ltd. should draw lessons from accidents, fully implement the main responsibility of enterprises for safe production, improve the ability and level of enterprises to prevent and curb production safety accidents, and prevent similar accidents from happening again. Put forward the following rectification measures:
(1) Strictly fulfill the main responsibility of production safety of production and operation units. We will conscientiously organize the study and implementation of General Secretary Xi Jinping's important expositions on safety production and laws and regulations such as the Safety Production Law of the People's Republic of China, so as to effectively improve personnel's awareness of safety responsibility and enhance the company's safety production management level.
(2) Comprehensively carry out safety risk identification. In accordance with the requirements of "Ningxia Hui Autonomous Region Safety Production Risk Management and Control and Safety Production Accident Hidden Danger Investigation and Management Measures", organize personnel to comprehensively investigate all kinds of safety risks existing in production and operation activities, revise the list of safety production risk classification management and control, and do a good job in production and operation activities. (3) Improve the safety production operation rules of enterprises. In view of the imperfect operation rules exposed by the accident, combined with the actual situation of the enterprise, the personnel were organized to revise the safety production operation rules. After the revision and improvement, special training will be organized for practitioners and strict implementation will be supervised.
(4) Promote the implementation of the safety production responsibility system for all staff. Combining with the actual situation of enterprises, we should standardize the establishment and allocation of post employees to prevent the lack of post safety responsibility due to heavy tasks, shortage of personnel and confusion of personnel.
(5) Educate and urge employees to strictly implement safety production rules and regulations. Strengthen the systematic training of safety production rules and regulations and strictly supervise the full implementation of safety production rules and regulations by employees, so as to prevent the occurrence of non-standard procedures for examination and approval of work tickets and the failure to wear and use labor protection articles in accordance with regulations.

The dual prevention mechanism of safety production risk grading control and hidden danger investigation and management has been established. According to the control list, a safety checklist is formulated, a safety hidden danger investigation and management account is established, a hidden danger management notice is issued for the problems found, and a closed-loop management is implemented in strict accordance with the "five definitions" principle of hidden danger rectification.
(3) Ningxia Jinyuyuan Resources Recycling Co., Ltd. organized personnel to revise the safety operation procedures for cleaning sludge in the concentration tank in view of the imperfect operation procedures exposed by the accident and in combination with the actual operation of cleaning sludge in the concentration tank, and organized personnel to carry out special training and strictly implement them.
(4) In February 2026, Ningxia Jinyuyuan Resources Recycling Co., Ltd. adjusted the company's management personnel, further clarified the safety production responsibilities of management personnel at all levels, revised and issued the Safety Production Responsibility System, matched the post employees, refined the post responsibilities and supervised the assessment.
(5) Ningxia Jinyuyuan Resources Recycling Co., Ltd. revised the safety management standards for special operations in August 2026, and handled special operation tickets in strict accordance with the management standards. Set up the safety regulations for the maintenance of the concentration tank in the production site, clarify the safety height parameters of the mud from the bottom of the tank before the maintenance of the tank and the regulations for the handling of the operation ticket, and put forward the requirements for the standard wearing of labor protection articles for the workers during the operation.
(6) Ningxia Jinyuyuan Resources Recycling Co., Ltd. The account of special operators has been established. Special operations such as equipment maintenance and temporary power utilization shall be carried out by qualified personnel.
5. Evaluation
of the Safety Status of Accident Enterprises Enterprises earnestly draw lessons from accidents, carry out safety education and training for all staff, and organize the study and implementation of General Secretary Xi Jinping's important expositions on safety in production and laws and regulations such as the Safety in Production Law of the People's Republic of China. Revise and issue the responsibility system for production safety and safety management standards for special operations, identify and update the list of risks in the workplace, carry out hidden danger investigation and management, strengthen the management of special operations in the production site, and standardize equipment inspection and maintenance. 6. By consulting the safety management and training education materials of the accident enterprise, the evaluation team of the education situation
of the accident enterprise inquired about the safety management personnel and front-line employees, carried out an accident review within the company after the accident, conducted warning education and training for all employees, and issued the accident investigation report to the workshop section.
