
The investigation pointed out that Yongan Jinniu Cement Co., Ltd., as the source enterprise of goods, basically performed its daily management in place, but there were problems of not strictly supervising the loading of trucks and releasing slightly overloaded vehicles out of the station. Although the slight overload has no direct causal relationship with the accident, it has constituted a potential safety hazard and violated the relevant requirements of cargo loading source management.
In view of the above problems, the accident investigation team asked Yongan Jinniu Cement Co., Ltd. to draw lessons profoundly, implement the responsibility system of safety production for all staff, organize all staff to study the Regulations on Overloading and Overloading of Highway Freight Transport Vehicles in Fujian Province, improve and strictly implement the management system of truck loading, earnestly fulfill the responsibilities and obligations of the source units, and prevent the occurrence of similar hidden dangers.

Investigation Report
on the "8.23" General Road Transport Accident of Datian Construction Co., Ltd. When Xu Yongwei drove Fujian B56682 heavy semi-trailer tractor to pull Fujian B3636 heavy tank semi-trailer carrying bulk cement powder to the section of Provincial Highway 217 12 km + 100 m (Jianzhen Jiancun), he collided with the three-wheeled electric vehicle with the number plate of "Sanming Sanyuan Temporary 06757" driven by Wu Liankui, who was turning left in the same direction ahead. The road traffic accident that caused Wu Liankui's death on the spot and the damage of two vehicles resulted in a direct economic loss of about 430000 yuan. On August 29,
2025, the Safety Production Committee of the People's Government of Sanming City issued to the People's Government of Datian County the Notice on the Listing Supervision of the Investigation and Disposal of the "8.23" General Road Transport Accident in Datian Construction (Ming'an Committee [2025] No.20), which implemented the listing supervision of the investigation and disposal of the production safety accident. In accordance with laws and regulations such as the Safety Production Law of the People's Republic of China, the Road Traffic Safety Law of the People's Republic of China, the Regulations on Reporting and Investigating Production Safety Accidents (Decree No.493 of the State Council), and the Regulations on Safety Production of Fujian Province, and with the consent of the People's Government of Datian County, the Emergency Management Bureau of Datian County shall organize the county. Datian Construction "8.23" General Road Transport Accident Investigation Team (hereinafter referred to as the Accident Investigation Team) was established, and Datian County Discipline Commission and Putian Xiuyu District Government were invited to intervene in the accident investigation. In accordance with the principles of "four not letting go" and "scientific rigor, compliance with laws and regulations, seeking truth from facts and paying attention to practical results", the
accident investigation team finds out the course, causes, emergency disposal, casualties and direct economic losses of the accident by means of on-site investigation, access to information, investigation and inquiry, inspection and identification, and technical analysis. The nature and responsibility of the accident were identified, the suggestions for dealing with the relevant units and responsible personnel were put forward, and the relevant preventive measures for the accident were put forward in view of the causes of the accident and the prominent problems exposed. According to the investigation by the
accident investigation team, the "8.23" general road transport accident of Datian Construction was a general production safety accident caused by a truck driver who did not turn on the left turn signal ahead of time and honked his horn, did not overtake after confirming that there was a sufficient safe distance, and the motorcycle driver did not turn on the left turn signal ahead of time when turning left.

accident units Fujian Changhong Logistics Co., Ltd. has 13 heavy semi-trailer tractors, 18 heavy semi-trailers and 13 drivers. Huang Zhaohua, the main person in charge, presided over the company's safety production work. Huang Zhaohua, the main person in charge, has obtained the certificate of safety assessment for the main person in charge of road transport enterprises, which is valid from September 25, 2020 to September 25, 2026. Qian Guohui, full-time security officer of Fujian Changhong Logistics Co., Ltd., male, born on August 12, 1960, native of Chengxiang District, Putian City, Fujian Province, ID No.: 35 * * * * * * * * * * * * 011. After the first training in 2023, he obtained the Certificate of Qualification for Safety Assessment of the Principal Responsible Persons and Safety Production Management Personnel of Road Transport Enterprises. The validity period is from November 22, 2023 to November 22, 2026.
Xu Yongwei, a driver of Fujian Changhong Logistics Co., Ltd., signed a labor contract with Xu Yongwei on January 5, 2021, and renewed the contract once a year (from January 2022 to January 2024). The current valid contract period is from January 1 to December 31, 2025.
(6) After
the accident, Xu Yongwei drove Fujian B56682 heavy semi-trailer tractor to pull Fujian B3636 heavy tank semi-trailer from Putian to Yongan Jinniu Cement Co., Ltd. to load bulk cement powder at about 4 A. M. on August 23, 2025. At 7:53, after Xu Yongwei loaded the bulk cement powder (the total weight of the vehicle and goods was 51.08: 11, Xu Yongwei drove to the straight dry cement pavement section of 12km + 880m (Jianshe Village, Jianshe Town) of Provincial Highway 217 in Datian County, and drove to the left side of the road. Futian Five-star Motorcycle with "Sanming Sanyuan Temporary 06757", driven by Wu Liankui without a license or helmet, was driving in the same direction in the same lane ahead of him, when Wu Liankui suddenly turned left, because Xu Yongwei did not turn on the left turn signal and honked his horn in advance when overtaking, and Wu Liankui suddenly turned left. As a result, Wu Liankui was crushed by the right front wheel of the heavy semi-trailer tractor after falling on the road, resulting in a road traffic accident in which Wu Liankui died and two vehicles were damaged.

of the accident The scene of the accident is located in the 12km + 880m straight dry cement pavement section of Provincial Highway 217 in Datian County. The road runs from north to south, south to Datian County and north to Sanming City. The total width of the road is 7.30 m, with a single dotted line in the center of the road and a marking on the edge of the road. The width of the road shoulder on the east side is 1.40 m, and the width of the road shoulder on the west side is 1. Outside the road on the west side are civilian houses, and outside the road on the east side are flower beds. There are speed limit signs for small vehicles with a speed limit of 60 km/H and medium and above vehicles with a speed limit of 50 km/H in the direction of Sanming on the 13 km + 600 m section of Provincial Highway 217. At the time of the accident, the weather was fine and the visibility was good.
(8) Casualties and direct economic losses
The accident resulted in one death and a direct economic loss of about 430000 yuan.
The deceased Wu Liankui, male, Han nationality, family address, No.198, Datong Village, Jianshe Town, Datian County, Fujian Province, "Sanming Sanyuan Temporary 06757" (invalid number plate), the owner of Futian Five-star three-wheeled motorcycle, did not obtain a motor vehicle driving license in accordance with the law, died on the spot when the accident occurred, and was cremated on August 25, 2025.
2. Emergency disposal and assessment
of accidents (1) Reporting and response of accident information. After
receiving the report, Datian County Public Security Bureau, Construction Town People's Government and other relevant departments immediately rushed to the scene to carry out safety protection and on-site investigation. Fujian B56682 heavy semi-trailer tractor (towing Fujian B3636 heavy tank semi-trailer) and "Sanming Sanyuan Temporary 06757" (invalid number plate) ordinary three-wheeled motorcycle were temporarily detained, preliminary investigation was carried out, relevant personnel were investigated and inquired, and accident information was reported according to regulations.
(II) Emergency disposal
at the scene of the accident At 8:13 on August 23, 2025, Xu Yongwei dialed the medical emergency telephone 120 and the alarm telephone 110 with his mobile phone (number: 136 * * * * 0300). 23, 120 ambulance workers from Datian Minsheng Hospital arrived at the scene and 120 doctors confirmed that motorcycle driver Wu Liankui had died. 25, the police of the construction police station of Datian County Public Security Bureau arrived at the scene of the accident, quickly controlled Xu Yongwei, the driver of the vehicle causing the accident, organized on-site safety protection and order maintenance, and immediately resumed traffic. 33, the traffic police brigade of Datian County Public Security Bureau arrived at the scene of the accident and organized on-site disposal work such as on-site investigation and preliminary inquiry of the accident. Start the emergency response in time and handle the emergency properly on site.
(III) After the
accident, with the joint efforts of Jianshe Town Government and Fujian Changhong Logistics Co., Ltd., Fujian Changhong Logistics Co., Ltd. paid the funeral expenses of 100,000 yuan to the family members of the deceased. At present, the insurance claim procedure is in the normal process. There was no mass petition incident in the accident, and the local social order was stable.
(4) Accident emergency rescue assessment
After the accident, the accident rescue responded quickly in advance, the scene was handled properly, the rescue operation was carried out in an orderly manner, the accident emergency disposal was in place, and no secondary accidents occurred, which conformed to the requirements of relevant laws and regulations and accident handling work norms.

The main problems of the above accident units have no causal relationship with the accident.
5. Opinions
on the handling of the responsible persons and units concerned (1)
Wu Liankui, male, is the owner of the three-wheeled ordinary motorcycle hanging the "Sanming Sanyuan Temporary 06757". Driving a three-wheeled motorcycle on the road without obtaining a motor vehicle driving license according to law and turning left without turning on the left turn light in advance violates Article 19, paragraph 1, of the Road Traffic Safety Law of the People's Republic of China and Article 57, paragraph 1, of the Implementation Regulations of the Road Traffic Safety Law of the People's Republic of China, respectively. He is one of the causes of the accident and is directly responsible for the accident. In view of his death in the accident, it is suggested that he be exempted from accountability.

In addition, according to the certificate of road traffic accident issued by the traffic management brigade of Datian County Public Security Bureau (No.041 of the 350425120), the accident was caused by Xu Yongwei driving a motor vehicle without turning on the left steering light and honking the horn in advance, overtaking after confirming that there was sufficient safe distance, and Wu Liankui did not obtain a motor vehicle driving license according to law. When turning left, the left turn signal was not turned on ahead of time, which caused the road traffic accident. There is no direct causal relationship between the accident and the safety management of Fujian Changhong Logistics Co., Ltd. It is suggested that no administrative penalty should be imposed on Fujian Changhong Logistics Co., Ltd. Huang Chaohua, the principal responsible person of Fujian Changhong Logistics Co., Ltd., and Qian Guohui, the full-time security officer, basically perform their duties in daily management, but they still need to be strengthened. It is suggested that no administrative penalty should be imposed on Huang Chaohua, the principal responsible person, and Qian Guohui, the full-time security officer.
Yongan Jinniu Cement Co., Ltd., as the source unit of cargo loading, basically performs its duties, and there is no strict supervision of trucks loading goods in the unit, and slightly overloaded vehicles are released from the station. It is suggested that Yongan Emergency Management Bureau should deal with them according to law. The Transportation Bureau of Xiuyu District
of Putian City did not give feedback on the supervision of Fujian Changhong Logistics Co., Ltd. and suggested that the government of Xiuyu District of Putian City further investigate and deal with the situation of the Transportation Bureau and its supervised enterprises.
VI. Accident Prevention and Rectification Measures
In order to prevent the occurrence of similar accidents, it is suggested that relevant units implement the following rectification measures:
(I) Fujian Changhong Logistics Co., Ltd
. Fujian Changhong Logistics Co., Ltd. should draw lessons from the "8.23" accident. Effectively implement the main responsibility of enterprise safety production: First, technical and management measures should be taken to strictly implement the hidden danger investigation system and vehicle maintenance management system; Secondly, we should strictly urge employees to learn traffic laws and regulations and strictly abide by traffic safety rules and safety operation rules, strengthen the assessment of safety rewards and punishments for employees, and increase the penalties for violations of laws and regulations. Thirdly, we should organize and carry out risk research and judgment, carefully sort out the risk points and weak links of the unit, and implement control measures in time for the problems sorted out by the research and judgment.
(II) Yongan Jinniu Cement Co., Ltd

Yongan Jinniu Cement Co., Ltd. should draw lessons from the "8.23" accident and earnestly implement the main responsibility of the enterprise for safe production: First, earnestly implement the responsibility system for safe production of the whole staff of the unit. In view of the illegal acts committed by the people in the jurisdiction in the process of driving motor vehicles, such as driving without a license and driving motor vehicles overdue for annual inspection, we should publicize them in time, strive to create a safe and good social environment, and effectively prevent and resolutely curb all kinds of accidents in production safety.
