2. Brief description
of the accident In the concrete pouring operation, a concrete pump truck collapsed due to insufficient bearing capacity of the foundation supported by the right front outrigger after being transferred to the new operation point, resulting in the instability and inclination of the pump truck boom, hitting Hu XX, a worker working on the second floor, and causing his death.
3. The direct cause
of the accident was the illegal operation of the concrete pump truck operator:
the pump truck outrigger was erected in serious violation of regulations (the skid was not used, the base plate was not placed in the forward direction, the outrigger was not fully extended, and the outrigger intruded into the scaffold area). The ground bearing capacity of the operation point
is not verified.
The accident checking calculation shows that the actual bearing capacity of the foundation at the accident site is far lower than bearing capacity required for the operation of the pump truck's outrigger.
IV. The main problems exposed by the accident and the responsible
construction unit (Guangdong Zhongqin Construction Group Co., Ltd.):
The main responsibility of safety production is not implemented, and the safety management system is a mere formality. No safety production management agreement has been signed
with the equipment leasing subcontractor, and the management is missing.
Safety education and training were not in place, and the deceased was a temporary worker without training. No special construction plan has been
prepared, and there are loopholes in technical management.
In order to catch up with the construction period and organize night operation in violation of regulations, the project manager, safety officer and other key personnel were not on duty at the time of the accident.
Equipment leasing unit (Shenzhen Luyuan Construction Equipment Leasing Co., Ltd.):
The bearing capacity of the foundation at the operation site has not been verified.
The safety operation procedures are not standardized, and the training of operators is not in place.
Acquiesce and fail to stop the illegal operation of the operator.
Supervision unit (Shenzhen Zhenqiang Construction Engineering Management Co., Ltd.):
failed to effectively perform its supervision duties, and failed to supervise the personnel management of the general contractor and the signing of the subcontracting agreement. The operating conditions after the displacement of the pump truck
were not re-checked and accepted. Effective on-site supervision has not been carried out
for high-risk operations at night, and hidden dangers have not been found and eliminated in time.
V. Handling and Rectification Measures
Handling of the Responsible Persons: It is suggested that the relevant responsible persons of the enterprises involved (such as the person in charge of the project, the safety officer, the person in charge of technology, the pump truck operator, the chief supervision engineer, etc.) should be punished by the Emergency Management Bureau or handled by the subordinate enterprises in accordance with the regulations. Handling of
responsible units: It is suggested that Guangdong Zhongqin Construction Group Co., Ltd. and Shenzhen Luyuan Construction Equipment Leasing Co., Ltd. should be subject to administrative punishment according to law; Shenzhen Zhenqiang Construction Engineering Management Co., Ltd. should be punished according to law. Accountability
to the supervision department: instruct the Management Committee of Boluo Industrial Park in Huizhou and the Housing and Urban-Rural Development Bureau of Boluo County to make a profound written inspection to the county government, and hand over the clues about the performance of public officials to the Commission for Discipline Inspection and Supervision.
Accident prevention measures: The report puts forward specific rectification and prevention requirements from three aspects of compacting the main responsibility of enterprises, strengthening precise supervision and law enforcement, and strengthening the construction of safety culture, aiming at preventing similar accidents from happening again.
