, 2026, a safety accident occurred during the cleaning operation of the mixing tank in the enterprise, resulting in the death of a worker and a direct economic loss of about 1.01 million yuan. The investigation report confirmed that the accident was a general accident of production safety responsibility, and several responsible persons were recommended to be held accountable, and the enterprises involved would face administrative penalties. , 2026, Xinjiang Shunbo Building Materials Co., Ltd. in Shanghu Town of Korla City carried out equipment cleaning work before resuming production. Technician Liu Jianxin arranged employee Eskar Sadik to enter the cement mixing tank for cleaning work. Guan Jun, an employee of the
enterprise, started the power supply of the control room without confirming the safety status of the personnel in the tank and without carrying out the equipment start-up safety confirmation process, causing Eskar Sadik in the tank to be squeezed by the mixing impeller. After the incident, the enterprise dialed the emergency telephone at the first time, and at 13:30, the healthcare workers arrived at the scene to confirm that the party had died. Subsequently, the enterprise reported to the police, the relevant departments reported the accident information step by step, and the enterprise reached a compensation agreement with the family members of the deceased in the aftermath stage, with a compensation amount of 1.01 million yuan. After the
accident, the Korla Municipal People's Government set up an accident investigation team on March 25, which was composed of the Municipal Commission for Discipline Inspection, the Procuratorate, the Emergency Management Bureau, the Public Security Bureau, the Federation of Trade Unions and the Shanghu Town Government. In strict accordance with the principle of "four not letting go", the accident was investigated through on-site investigation, inquiry and evidence collection, and technical research and judgment.
The investigation team found that Guan Jun's illegal start-up equipment was the direct cause of the accident. Multiple management loopholes are the indirect causes of the accident: the enterprise did not carry out safety education and training for new employees, and the operators did not grasp the post risks; the cleaning operation of the mixing tank did not implement the safety measures of power off, listing and locking; the enterprise lacked the special safety operation rules for the inspection and maintenance of the mixing tank, and the hidden danger investigation and management system became a mere formality. At the same time, there are many outstanding problems in the enterprise, such as the inconsistency between the registered address and the actual business address, the failure to sign labor contracts with employees, the lack of safety warning signs for machinery and equipment in the factory area, and the failure to report contingency plan.
The investigation report clarifies the handling opinions of relevant personnel: Guan Jun, the operator who caused the accident, is suspected of committing a criminal offence, and it is suggested that he should be handed over to the judicial organ for investigation of criminal responsibility; Liu Jianxin, the on-site technician, violated the rules and arranged dangerous operations orally, and did not arrange on-site monitoring, which should be severely handled within the enterprise; Guan Li, the legal representative of the company, as the main person in charge, is absent in the performance of safety production management and bears important leadership responsibility for the accident, which shall be punished by the emergency management department according to law. Xinjiang Shunbo Building Materials Co., Ltd. is facing administrative penalties for a number of safety production violations. At the same time,
the report points out the shortcomings of the supervision level: there are loopholes in the safety supervision of the industrial and trade enterprises of the Emergency Management Bureau of Korla City; the people's government of Shanghu Town is affected by the change of the business address of the enterprise, and there is a lag in the safety inspection. The two units are instructed to make a written inspection to the Korla Municipal Government and hold the relevant staff accountable.
Cement mixing tank cleaning and limited space maintenance have always been high-frequency risk operations in concrete and building materials industries. The accident exposed the common hidden dangers of small and micro enterprises: the dangerous operation process exists in name only, the locking and listing system is not implemented, the oral dispatch, the lack of special supervision, and the lack of personnel safety training. The investigation team requires the enterprises involved to rectify immediately and comprehensively, improve the system of examination and approval of limited space operations and confirmation of equipment start-up, and carry out safety warning education regularly.
In view of the safety management and control of the industry, the report proposes measures to strengthen supervision. The regulatory authorities will intensify the investigation of small and micro building materials enterprises in their jurisdiction, implement differentiated inspection according to the risk level, and improve the closed-loop mechanism of hidden danger investigation, rectification and review. Territorial villages and towns should comprehensively arrange the main body of production and operation in their jurisdictions, accurately grasp the information of enterprises registered in different places and operating on the spot, and eliminate the blind areas of supervision.
