evaluation working group carefully combed the basic situation, causes, nature, responsibility identification, treatment suggestions and rectification measures of the accident in the "7.10" General Object Strike Accident Investigation Report of the Sand and Aggregate Production Line of Jianchuan Cement P lant (hereinafter referred to as the report). On the basis of supervising the follow-up treatment of the accident organized by the County Security Committee Office, members of the expert group were invited to conduct in-depth accident assessment by consulting the original accident files, relevant documents and materials, on-site inspection and listenin G to reports. In this accident, the implementation of the handling opinions of the relevant responsible units and responsible personnel, the rectification of the problems exposed in the accident, the lessons learned from the accident and the implementation of the rectification measures in the local towns were evaluated. The evaluation team conducted a careful evaluation and formed an evaluation report on the accountability of the accident and the implementation of the rectification measures.

In violation of the provisions of Article 21, paragraph 5, of the Safety Production Law of the People's Republic of China, it is suggested that the Emergency Management Bureau of Jianchuan County impose a fine of 40% of the annual income of the previous year in accordance with the provisions of Article 95, paragraph 1, of the Safety Production Law of the People's Republic of China. It violates the provisions of Article 25, Paragraph 1, Items 2 and 3 of the Safety Production Law of the People's Republic of China, and suggests that the Emergency Management Bureau of Jianchuan County impose a fine of 20% of the annual income of the previous year in accordance with the provisions of Article 96 of the Safety Production Law of the People's Republic of China.

Handling and implementation: The Emergency Management Bureau of Jianchuan County has imposed a fine of 20% of the annual income of the previous year according to the Decision on Administrative Penalty (Individual) (Jian Emergency Penalty [2025] No.12-3). That is, RMB 18384.

3. Implementation of
responsibility investigation of accident responsible units (1) Jianchuan Cement Plant. The main responsibility of enterprise safety production is not in place, the responsibility of safety production management in the responsibility system of safety production is not effectively fulfilled, the site safety management is not in place, the identification of potential safety risks is insufficient, and the elimination of potential safety risks is not timely, which leads to the occurrence of this general production safety responsibility accident. Their actions violate the provisions of Articles 41 and 44 of the Safety Production Law of the People's Republic of China.
Handling suggestion: It is suggested that the Emergency Management Bureau of Jianchuan County shall impose administrative penalties on the illegal acts of Jianchuan Cement Plant in accordance with Item 1, Paragraph 1, Article 114 of the Production Safety Law of the People's Republic of China and Item 2, Article 14 of the Regulations on Fines and Penalties for Production Safety Accidents (Order No.14 of the Ministry of Emergency Management).
Handling and implementation: The Emergency Management Bureau of Jianchuan County has imposed an administrative penalty of RMB 500,000 according to the Administrative Penalty Decision (Unit) (Jian Emergency Penalty [2025] No.12), and the fine has been collected to the State Treasury.
(2) Jianchuan Taiyu Security Service Co., Ltd. Safety training and education for labor dispatchers are obviously inadequate.

Handling and implementation: The Human Resources and Social Security Bureau of Jianchuan County interviewed Taiyu Security Service Co., Ltd. of Jianchuan County in the Labor Contract Law of the People's Republic of China on October 28, 2025. According to Paragraph 2 of Article 92 and Paragraph 1 of Article 8 of the Interim Provisions on Labor Dispatch, Taiyu Security Service Co., Ltd. of Jianchuan County was ordered to check and fill in the gaps, improve the induction training system, comprehensively investigate the weak links in the company's safety production and management, timely complete the annual duty of induction knowledge and safety education and training for all dispatched workers for different types of work, take the case as a lesson, draw profound lessons and take the initiative to compact. 5. Implementation
of rectification measures for accident investigation report (1) Implementation of rectification measures

First, in view of the outstanding problems exposed by the accident, they immediately carried out in-depth investigation and rectification of safety production in the industrial and trade industry throughout the county. Focus on high-risk scenarios such as high-risk operations, equipment inspection and confined space, focus on key links such as outsourcing, inspection and maintenance, strengthen risk prediction and closed-loop management of hidden dangers, and resolutely implement the "five implementations" of rectification measures, responsibilities, funds, time limits and plans. The second is to compact the responsibilities of all parties. Strengthen coordination and cooperation among regulatory departments, further clarify and strictly implement regulatory responsibilities, enrich front-line regulatory forces, and enhance professional regulatory capabilities. Through frequent safety inspections of enterprises in the jurisdiction, we urge enterprises to implement their main responsibilities, establish and improve the working mechanism of supervision and inspection of enterprise safety work, urge the principal responsible persons of enterprises to strictly perform their statutory duties, increase safety investment, improve management system, strengthen safety management personnel, and strengthen risk classification control and hidden dangers. At the same time, we should actively promote the implementation of territorial responsibilities, effectively strengthen communication, coordination and guidance to villages and towns, promote the implementation of "the same responsibility of the Party and the government, one post and two responsibilities" and territorial management responsibilities, and form a joint force of supervision. Third, strengthen the construction of safety infrastructure. We will vigorously promote the upgrading of mechanization, automation and intellectualization of enterprises, and eliminate backward technology and equipment. Strictly control the access of employees, urge enterprises to strictly implement the three-level safety education and training system, especially to strengthen the training and assessment of front-line employees'post risk identification and emergency response capabilities, so as to ensure the effectiveness of training. To guide enterprises to improve contingency plan, strengthen emergency drills, and enhance on-site emergency response capabilities. Fourth, we should seriously carry out warning education. Taking this accident as a typical case, in view of the problems exposed in the accident, warning education was carried out in the county's industrial and trade industry supervision departments and enterprises to alert all parties with blood lessons, and to enhance the sense of responsibility and urgency for safe production.
To sum up, the assessment team believes that the relevant responsible units and personnel of the accident have basically implemented the handling suggestions proposed in the accident investigation report; the rectification measures proposed in the accident investigation report have been basically implemented; Accident-responsible enterprises and territorial townships can earnestly draw lessons from accidents, draw inferences from each other, pay attention to strengthening enterprise safety management, and strengthen the implementation of safety responsibilities.
6. Opinions and suggestions
Jianchuan Industrial Information and Science and Technology Bureau, Jianchuan Emergency Management Bureau and Jinhua Town People's Government should further strengthen the safety supervision of local enterprises within their responsibilities, improve relevant systems and work measures, constantly improve the pertinence and effectiveness of the work, and earnestly grasp the work of safety production.
