There was no on-site monitoring for night unloading, and the behavior of operators was not controlled, resulting in problems such as unauthorized unloading by drivers and illegal detention in dangerous areas. 3. The vehicles and drivers involved are all personnel of the outsourcing unit, and the enterprise has not incorporated the outsourcing team into the unified safety management and control system of the whole plant, and has not carried out effective safety training and education for the operators of the outsourcing unit. The operators of the outsourcing unit are not familiar with the Safety Operation Rules for Vehicles in the Raw Material Yard, the division of dangerous areas and the knowledge of emergency avoidance, and their safety awareness is weak, resulting in illegal operations. As the direct person in charge of workshop safety, he failed to strictly implement the closed-loop management and control of enterprise operation rules, and neglected the hidden dangers of forklift drivers working without certificates, unloading operations without special supervision, and super-high and super-steep stacking of carbide slag, resulting in production safety accidents.
