Recently, Changsha County People's Government officially announced the investigation report on the "3.29" general asphyxiation accident of Hunan Yinshantai Cement Co., Ltd. According to
the investigation report, at 14:38 on March 29, 2025, a general asphyxiation accident occurred in Hunan Yinshantai Cement Co., Ltd. (Hereinafter referred to as "Yinshantai Company") located in Jiangbei Town, Changsha County, resulting in the death of a 58-year-old employee of the company and a direct economic loss of 1.29 million yuan. After the accident, the relevant leaders of the emergency management departments at the provincial, municipal and county levels rushed to the scene to guide the rescue and rehabilitation work, and the Municipal Security Commission Office supervised the investigation and handling of the accident.
After verification, the accident occurred in the company's No.31 fly ash silo, which is a cylindrical concrete structure with a height of 30 meters, a width of about 12 meters and a capacity of about 1200 tons. It has been completely closed and abandoned for 10 years, and the pipelines have been cut off and stopped. Due to the large amount of residual waste in the warehouse affecting load-bearing and potential safety hazards, Yinshantai Company decided to clean it up, and issued a cleaning notice on March 24, and formulated a clearance plan on March 25. The direct cause of
the accident has been identified: the employees involved had weak safety awareness, violated the safety regulations for warehouse clearance operations, failed to implement the "top-down" principle of warehouse clearance, failed to control the surplus length of the safety rope within 1 meter during the operation, and stood on the caked fly ash to use the air pick machine, resulting in caking collapse, and the employee was buried and suffocated to death. De
eper indirect reasons expose serious loopholes in enterprise safety management. The investigation report pointed out that the on-site management of Yinshantai Company was not in place, the clearance plan was not formulated strictly in accordance with the Safety Regulations for Manual Clearance of Cylindrical Storage in Cement Plants, the operation process was not standardized, the over-age personnel were arranged to engage in high-risk clearance operations in violation of regulations, and the supervisors were not sufficient; At the same time, the safety education and training of enterprises and emergency drills before operation are not in place, and the operators and guardians are not familiar with the safety regulations of warehouse clearance, which lays a major hidden danger for accidents. After the
accident, on-site guardians and relevant personnel of enterprises quickly carried out rescue, dialed 120 emergency calls and implemented cardiopulmonary resuscitation, rescue response was rapid and properly handled, and no secondary accidents occurred. On March 30, Yinshantai Company reached an agreement with the family members of the deceased, and all the related aftermath matters were fulfilled. After investigation, the territorial government and relevant regulatory authorities have fulfilled their duties of safety production supervision, and no inadequate performance has been found.

In accordance with the Production Safety Law of the People's Republic of China, the Regulations on the Reporting, Investigation and Handling of Production Safety Accidents and other relevant laws and regulations, and in combination with the cause of the accident and the determination of responsibility, the accident investigation team shall make a clear decision on the handling of the responsible units and the relevant responsible persons.
For the responsible unit, Yinshantai Company, due to inadequate safety management, did not regularly educate and urge employees to strictly implement safety production rules and regulations and safety operation rules, did not ensure the implementation of safety measures in place, and was mainly responsible for the accident. It is suggested that Changsha County Law Enforcement Bureau should impose administrative penalties according to law.
For the responsible person, the employee involved is the main person responsible for the accident, but in view of his death in the accident, he shall not be punished; The remaining five responsible persons were seriously held accountable, including: the general manager (actual controller) of the company failed to supervise and inspect the work of safety production, eliminate the hidden dangers of accidents in time, bear the main leadership responsibility, and was recommended to be given administrative penalties according to law; The director of the production department is not strict in the examination and verification of the inventory plan, the risk control is not in place, the director of the safety and environmental protection department is unreasonable in organizing and formulating the inventory plan, and fails to perform the duty of on-site inspection. Both of them are responsible for the management and are recommended to be given administrative penalties according to law; The workshop director was advised to be dealt with by the enterprise according to the internal rules and regulations because of inadequate on-site safety management, inadequate safety education and training, and the full-time safety officer was not familiar with the inventory clearance procedures and did not stop the illegal operation.
