2025, the People's Government of Fengrun District issued the Investigation Report on the "5.22" General High Fall Accident of Tangshan Branch of Tangshan Jidong Cement Co., Ltd. 15 on May 22,
2025, Wang Mou, the head of the cement process of the company, fell to his death while working in dangerous areas in violation of regulations, resulting in a direct economic loss of 1.97 million yuan.
Recently, the accident investigation team of Fengrun District Government of Tangshan City issued a special investigation report, which restored the whole process of the accident, identified that the accident was caused by the absence of enterprise safety management and the illegal operation of operators, and clarified the relevant accountability and rectification requirements. The
investigation report details the complete chain of the accident. At 11:02 on May 22, 2025, Mr. Hong, deputy director of the Production and Operation Department of the First Plant of Jidong Cement Tang Branch and the head of the cement delivery process, arranged Mr. Wang, the team leader, to go to the scene to take photos, and clearly informed him of the safe route: enter the corridor from the south side of the top of the clinker bin in the second-line cement batching station. After walking down the stairs for two sections, the shooting is completed on the stairs, and it is necessary to return the same way. After
Wang arrived at the shooting position according to the designated route, he did not comply with the operation requirements, walked down an extra staircase without authorization, climbed over the guardrail between the staircase and the roof of the electrical room to enter the roof of the electrical room, and completed the shooting at 11:09 and gave feedback to Hong. However, when he returned, Wang again illegally climbed over the guardrail of the patterned plate steel platform connected with the second-line 2B mill on the west side of the roof of the electrical room. When he stepped on the first piece of patterned plate steel on the platform, the welds on the east, south and north sides of the plate suddenly cracked, and only the west side remained connected, forming a collapse gap of 3 meters long and 1 meter wide. Wang immediately fell from 14.5 meters to the ground and was injured. The
site of the accident is located in the checkered plate steel platform between the 2B electrical room of Jidong Cement Tang Branch and the 2B cement mill of the second line. The on-site investigation shows that the platform is equipped with a 1.1-meter-high guardrail, but there are hidden quality hazards in the welded seam of the checkered plate steel involved, which eventually causes the accident due to the failure of bearing. After the
accident, the on-site emergency disposal work was carried out step by step. At 12:50, Xu Moumou, a part-time safety officer of the company, was looking for Wang Mou when he found him injured and fell to the ground at the place where he fell. He immediately reported to Hong Moumou and dialed 120 emergency calls. After Hongmou arrived at the scene, he reported to Wumou, Vice Minister of Operation Department of No.1 Production Plant, at 13:02. Wumou then reported to Xie, the manager of the company, at 13:30. Xie authorized Safety Minister Zheng to report the accident to Fengrun District Emergency Management Bureau at 14:23, and the District Emergency Management Bureau submitted written reports to Fengrun District Committee, District Government and Tangshan Emergency Management Bureau at 14:50. In terms of
medical rescue, the ambulance arrived at the scene at 13:20, and the healthcare workers sent Wang to the Second People's Hospital of Fengrun District for treatment after first aid. The relevant person in charge of the Emergency Management Bureau of Fengrun District led a team to the scene to verify the situation, ordered the enterprise to stop all operations immediately, comprehensively investigate potential safety hazards, and demanded that all efforts be made to comfort family members and deal with the aftermath. After comprehensive investigation and evidence collection by the
investigation team, it was determined that the accident was a production safety responsibility accident, and the core reasons involved two aspects of enterprise management and operator operation:
on the one hand, the main responsibility of safety production of Jidong Cement Tang Branch was not in place. Although the enterprise has set up a safety production department, equipped with 10 full-time safety officers, formulated the responsibility system, rules and regulations and contingency plan, there are obvious loopholes in the actual implementation: the lack of pertinence of safety production education and training, the lack of effective promotion of risk prevention awareness of operators; Risk control and hidden danger investigation and management are not in place, and the hidden dangers of welding seam safety of the pattern plate steel platform are not found and eliminated in time, and the safety warning and protective measures in dangerous areas are not effectively restrained.
On the other hand, Wang Mou, an operator, has a weak sense of safety, and illegal operation is the direct cause of the accident. In the case of receiving clear safety route guidance, he crossed the guardrail twice without authorization and entered the non-designated operation area, which violated the safety production rules and regulations and operation rules of the enterprise, and lacked sufficient awareness of his own safety and site risks. According to the principle of "Four Not-Let-Go", the
accident investigation team put forward targeted handling suggestions: the relevant management personnel and responsible persons of Jidong Cement Tangshan Branch who are responsible for the accident shall be held accountable in accordance with the law and regulations; the accountability team of the District Discipline Inspection Commission shall carry out investigation simultaneously, and the relevant units and personnel who fail to perform their duties shall be held accountable seriously.
At the same time, the investigation report clarifies the preventive and rectification measures of enterprises: First, strengthen safety production education and training, focus on dangerous operation links to carry out special training, enhance staff safety awareness and standardized operation ability; The second is to carry out comprehensive investigation and management of potential safety hazards, focusing on special inspection and maintenance of high-altitude operation platforms, guardrails, welds and other facilities, and establish a hidden danger account and closed-loop management; the third is to strictly manage dangerous areas, improve safety warning signs, strengthen on-site monitoring, and eliminate illegal entry into dangerous areas; Fourth, we should improve the responsibility system for production safety, strengthen the assessment of system execution, and ensure that all safety measures are in place.
