Safety production management of
Fenggu Cement Company Due to problems such as arrears of workers' wages, suppliers' payment for goods and bank loans, Fenggu Cement Company suspended production throughout 2024 and resumed production in March 2025. After resumption of production, the main production is processing with supplied materials. That is to say, the solid ore and waste materials of customers are crushed and processed. The crushing process flow is as follows: the ore is primarily crushed by a 600 * 900 jaw crusher (commonly known as a large machine), and then sent to a hammer crusher by a conveyor belt for secondary crushing, and at the same time, primary separation is carried out (there is a magnet in the middle of the conveyor belt for iron picking and impurity removal). And then transported to a 400 * 600 jaw crusher and a 250 * 1200 jaw crusher (commonly known as a 1-meter crusher) for crushing again to sort out particles with different diameters.
Since Fenggu Cement Company resumed production in March 2025, there are 17 employees working in two shifts. Safety management personnel include legal representative Cao Mou-mou, factory director Lu Mou-mou, safety officer Cao Mou-mou (and forklift driver), specifically, Lu Mou-mou is responsible for the ball mill workshop, Cao Mou-mou is responsible for the crushing workshop, Cao Mou-mou is responsible for the general, all of them have not obtained the qualification certificates of the main responsible persons and safety production management personnel. Special operators include electrician Zhang Mouzhu, who is a temporary external employee and has obtained the electrician certificate; mechanic Wu Mouzhu, an employee of the factory, has not obtained the welding and thermal cutting operation certificate. The safety production management organization and safety production responsibility system are not perfect, the factory director and safety officer are appointed orally by Cao, the safety management personnel are not fully equipped, the responsibilities are not clear, the safety production operation rules are not established, the safety production education and training for employees are not carried out, the hidden danger investigation and management are not in place, and the emergency drill is not carried out.
The accident happened on
the night shift on July 17, 2025 (from 20:00 on July 17, 2025 to 8:00 on July 18, 2025). There were three people on duty in the crushing workshop: forklift driver Liu Moumou, crusher worker Yang Mouwen and crusher driver Yang Mouqiao (female). Among them, Yang Mouwen and Yang Mouqiao were husband and wife. At 20:00, three people arrived at their posts and began to work: Liu was responsible for driving a forklift to feed the crusher, Yang was responsible for switching the crusher on and off at the control console of the 600 * 900 jaw crusher (commonly known as the large machine), and Yang was responsible for picking iron and removing impurities at the magnet in the middle of the discharge conveyor belt of the 600 * 900 jaw crusher. 00 on the
18th, Yang Mouwen found that the crusher was out of order and was not easy to use, so he shut it down and informed the legal representative Cao Mou of the situation, and then Cao Mou called the mechanic Wu Mou to go to the workshop to repair it as soon as possible. Wu immediately got up after answering the phone and rushed to the crushing workshop from the staff dormitory. After arriving at the workshop, he asked Yang to turn on the machine for inspection. After inspection, it was confirmed that the fault was the screw loosening of the fixed plate of the 250 * 1200 jaw crusher (commonly known as the 1-meter machine). Then Wu asked Yang to stop the machine and start maintenance. He first removed the side plate of the 1-meter machine, then called Zhang Moudong, the driver of the excavator, to lift the side plate, remove the two loose screws of the fixed plate, and go to the open space next to the conveyor belt of the large machine (also next to Yang Mouwen's station) to process the screws of the fixed plate. At this time, Zhang Moudong, the driver of the excavator, has left the workshop. At about
1:28, Wu Moumou began to process the screws of the fixed plate. Because the light at the processing site was not bright enough, Yang Mouwen took out the lamp hanging on his workstation (that is, the hook above the magnet in the middle of the discharge conveyor belt of the large machine) from the hook and put it on the ground of the processing point to strengthen the lighting. Then Yang Mouwen and Yang Mouqiao left the workshop for supper. Wu Mou-mou first cut two iron plates, then welded the screws on the iron plates, and fixed the screw processing operation at about 1:47 (during this period, the forklift driver Liu Mou-mou held the lighting lamp, that is, the lighting lamp on Yang Mou-wen's workstation, to strengthen the lighting, and put it back to the ground of the processing point after the screw processing operation). Then Wu Mou-mou punched the processed screws of the fixed plate into the screw holes of the 1-meter machine one by one, and welded and fixed the screws of the fixed plate from 2:13 to 2:35. Then Wu Mou-mou, Liu Mou-mou and Yang Mou-wen (Yang Mou-wen and Yang Mou-qiao had returned to the workshop) lifted the side plate onto the 250 * 1200 jaw crusher (commonly known as the 1-meter machine) for installation. After installation, Yang Mouwen and Yang Mouqiao started the machine to confirm that there was no problem. Wu Mouwen turned off the power supply of the welding machine, packed up the cutting and welding tools and left the workshop. Three people on duty returned to their respective stations to resume normal work. At about
2:42, when Yang Mouwen wanted to hang the lamp still at the processing point back to its original place (the hook above the magnet), his right thumb and forefinger touched the bare wire of the lamp wiring cable (the first knuckle of the right thumb and forefinger had burn marks, and there were no other scars all over his body), and he fell to the ground by electric shock (lying on his back). Head to the conveyor belt distance of more than 1 meter), Yang Mouqiao shouted "electric shock, people fell down", and ran from the large machine station to Yang Mouwen fell down. After hearing the shouting and seeing Yang Mouwen fall to the ground, Liu Mou-mou immediately ran to the main switch 10 meters away to close the power supply of the whole workshop. Then, together with Yang Mouqiao, Yang Mouwen was carried to an open place to press the chest for rescue. At this time, Yang Mouwen was still moving and called Cao Mou at the same time. 46, Xu Caomou rushed to the scene to organize rescue (cardiopulmonary resuscitation) and dialed 120 emergency calls. According to the on-site investigation, the accident occurred in the crushing workshop in the southwest corner of Fenggu Cement Company, which is a cuboid steel structure shed with a length of about 40 meters, a width of about 30 meters, a height of about 15 meters and four walls. The door is opened in the east; a 600 * 900 jaw crusher (commonly known as a large machine), a magnet, a hammer crusher and a 400 * 600 jaw crusher are installed in turn from north to south against the wall in the west, and are connected by a conveyor belt in the middle, with a total length of about 15m; Two conveyor belts are installed side by side against the wall in the south. The inner conveyor belt has a 250 * 1200 jaw crusher (commonly known as a 1-meter crusher) in the middle and is connected to a 400 * 600 jaw crusher. The outer conveyor belt is connected to a hammer crusher, with a total length of about 15 meters. A small amount of crushed finished products are piled up in a conical shape below the east end of the conveyor belt; The equipment is distributed along the west wall and the south wall in a 7-shaped connection; ore raw materials are piled up against the wall in the north, with a length of about 15 meters, a width of about 3 meters and a height of about 1.5 meters; In the middle of the workshop, two piles of finished products are piled up in a conical shape in the east. on punishment of the accident unit and relevant responsible personnel 1. Failure to improve and implement the safety production responsibility system of the whole staff of the unit; failure to formulate and implement the safety production rules and regulations and operating procedures of the unit; failure to strictly implement the safety production education and training system; Failure to strictly implement the risk provention and control and hidden danger investigation and treatment system; failure to guarantee the enterprise's investment in production safety, in violation of the provisions of Article 22 [①], Article 28 (1) [②], Article 41 (1) and (2) [③], Article 44 [④] and Article 46 [⑤] of the Production Safety Law of the People's Republic of China, and bear the main responsibility for the occurrence of the accident.
