At 7:08 on July 8, 2025, an asphyxiation accident occurred in a confined space during the sewage well testing work of the municipal infrastructure project of Wujin Changdao, Wujin Street, Xinjin District, Chengdu City, resulting in three deaths. In order to strictly implement the responsibility for production safety, seriously handle accidents, give full play to the role of accident warning education, and prevent similar accidents, according to the provisions of the Interim Measures for the Investigation and Supervision of Production Safety and Quality Accidents in Housing and Municipal Engineering, please do the following work well:

1. Deeply draw lessons from accidents, draw inferences from each other, strengthen the safety supervision of municipal engineering construction, comprehensively investigate potential risks, and resolutely prevent the occurrence of mass casualties.

2. Do a good job in accident investigation, find out the course and cause of the accident, put forward suggestions for improvement, form accident files containing pictures, images and other information, and upload the accident investigation report and warning education video data to the national engineering quality and safety supervision information platform before September 8, 2025.

3. Administrative penalties such as temporary withholding or revocation of safety production licenses, safety production assessment certificates and operation qualification certificates of special operators shall be imposed on responsible enterprises and personnel in accordance with the law and regulations, and the penalty information of the accident shall be uploaded to the national engineering quality and safety supervision information platform before October 8, 2025. If the enterprise qualification and personnel qualification issued by the Ministry are involved, the punishment suggestions shall be submitted to the Ministry in time according to the accident investigation report.
