2025, the People's Government of Xiangfen County approved the investigation report on the "6.17" general electric shock accident of the 5MW granulated slag roof distributed photovoltaic power generation project of Xiangfen Xingyuan Group Cement Building Materials Co., Ltd. The accident occurred at 11:15 on June 17, 2024. In the process of dismantling the steel ladder in the three-warehouse shed of the 5MW granulated slag shed roof distributed photovoltaic power generation project of Xingyuan Cement Building Materials Co., Ltd. in Xiangfen County, the steel ladder tilted and touched the 10 KV high-voltage overhead power line, resulting in one death and two injuries. After the
accident, the leaders of the Xiangfen County Party Committee and the county government attached great importance to it, demanding that the causes be found out as soon as possible, and that lessons be learned to prevent such accidents from happening again. On June 19, 2024, in accordance with relevant laws and regulations, the Xiangfen County People's Government immediately set up a "6.17" accident investigation team led by the County Emergency Management Bureau and composed of the County Public Security Bureau, the County People's Social Bureau, the County Industry and Information Bureau, the County Health and Sports Bureau, the County Energy Bureau, the County Electric Power Company, the Jingmao Township Government, the County Federation of Trade Unions and the County Discipline Commission to investigate the accident. Through on-site investigation, investigation and evidence collection, consulting relevant information and inquiring relevant personnel, the
accident investigation team found out the course, type and cause of the accident. After investigation, the accident was a general production safety responsibility accident caused by inadequate enterprise safety management and hidden danger investigation, indifferent safety awareness of construction personnel, low safety risk identification ability, and failure to take corresponding safety construction measures when dismantling steel ladders. , Xiangfen Xingyuan Group Cement Building Materials Co., Ltd. has some problems, such as lax examination and verification of the Construction Plan compiled by Hebei Sijian and inadequate safety inspection of the construction site. As the construction unit of photovoltaic power generation project, Hebei Fourth Construction Engineering Co., Ltd. has many problems, such as failing to implement the main responsibility of the enterprise, subcontracting the installation of steel structure and color steel plate to individual labor contractors without construction qualification, inadequate education and training of construction personnel, and inadequate safety management of construction site. In addition, Wan Baoping and Xu Huabin, the individual labor contractors of photovoltaic power generation projects, are engaged in labor contracting as individuals, and Xu Huabin organizes workers to work in violation of regulations without obtaining construction qualifications and signing construction contracts.
In the aspect of accident scene investigation, the surrounding environment and site conditions of the accident area, the accident steel ladder and installation, the status of other facilities on the site, and the disassembly plan were investigated in detail. The technical analysis of the accident shows that the stability analysis of the steel ladder in the static state after landing, the location analysis of the discharge traces, and the analysis of the electric shock injury are consistent with the actual situation of the accident.
In terms of casualties, aftermath and direct economic losses, the deceased Li Jianjie, male, 29 years old, died of electric shock; the injured Wu Shuiyan, male, 62 years old, injured his right arm; the injured Li Yinliang, male, 52 years old, is currently under treatment. The treatment of the aftermath includes signing a compensation agreement with the family members of the deceased and compensating the injured for the corresponding amount. The total direct economic loss is 1.69 million yuan.
In terms of handling suggestions for relevant responsible personnel and units, corresponding handling suggestions are put forward for responsible personnel such as Li Jianjie, Wan Baoping and Xu Huabin, as well as responsible units such as Hebei Fourth Construction Engineering Co., Ltd. and Xiangfen Xingyuan Group Building Materials Co., Ltd. It is suggested that the units and personnel responsible for supervision should be handled by the County Discipline Commission in accordance with the administrative authority of cadres.
In terms of the proposed preventive measures, Hebei No.4 Construction, as the construction contractor, must draw lessons from the accident and strictly implement the national safety production laws and regulations; Xingyuan Cement, as the project contracting unit, must strictly perform the safety supervision and management responsibilities of the contracting unit. In its reply,
the Xiangfen County People's Government said that the accident investigation conformed to the relevant laws and regulations, and agreed in principle with the accident investigation team's analysis of the cause of the accident and the identification of the nature of the accident. Agree with the opinions of the accident investigation team on the handling of the responsible persons and units, and investigate the responsibility of the responsible persons and units in accordance with the law and discipline.
Agree to the accident prevention measures and suggestions put forward by the accident investigation team, and the relevant units should earnestly draw lessons from the accident, strictly implement the preventive measures and suggestions, and further strengthen the work of safety in production. The county public security Bureau shall, in accordance with the law and regulations, immediately investigate the responsibility of the relevant personnel in accordance with the procedures;
the county emergency Bureau shall, in strict accordance with the work responsibilities and procedures, do a good job in the follow-up work. The rest of the relevant departments shall, in accordance with the statutory authority and procedures, do a good job in implementing the handling opinions of the responsible personnel and units, and publish the handling opinions to the public. The implementation of accident handling should be reported to the county emergency management Bureau in time.
