According to the accident investigation report, after two workers of Ruiding Company cleaned up the accumulated materials, they informed the dispatching room that the operation had confirmed that no one was around the belt corridor. After the operation of the belt, the on-site personnel found that there was an abnormality. After inspection, it was found that an outsourcing personnel was stuck between the belt and the counterweight wheel. After being rescued, the outsourcing personnel died after rescue.
In addition, Ruiding Company did not train the safety technical disclosure list and directly handed it over to Jiuli Labor Service Company for signature; the site management was chaotic, and the maintenance personnel entered the operation area without authorization. I. The course of the accident and the emergency disposal
(I) The course of the accident. On March 31,
2023, Hu Yong, the site manager of Ruiding Company, notified Zhang Chun, the construction manager of Jiuli Labor Service Company, by telephone to inspect and repair the belt tail wheel of Line 2 of Ruiding Company on April 1. At about 8:20 on April 1, 2023, Kang Zhonggang, the head of Jiuli Labor Service Company, had just finished the pre-shift meeting and arranged Wu Xiukun to prepare the gourd and steel cable. Subsequently, Kang Zhonggang went to the dispatching room to go through the examination and approval procedures of hot work tickets, power outage, plug pulling and listing. At 8:25, Wu Xiukun entered the tail wheel of the feeding belt of Line 2 of Lotus Pond Mixing Station of Ruiding Company to carry out the preparatory work before the construction of inspection and maintenance. At 8:30, Jiang Kekai, the operator of the dispatching room of Ruiding Company, started Line 1 after confirming that there was no one through walkie-talkie and video surveillance. At about 8:31, Wu Xiukun climbed up the belt counterweight drum of Line 2 alone without receiving instructions from Kang Zhonggang and without a guardian on the spot. At 8:20
on April 1, Ma Rong and Tao Jianbin of Ruiding Company carried out daily work on Line 2. After cleaning up the accumulated material on the belt tail wheel of Line 2, Ma Rong contacted Jiang Kekai, the operator of the dispatching room, through the walkie-talkie, and informed Jiang Kekai that he had checked and confirmed that there was no one around the belt corridor. After confirming with Ma Rong at 8:32, Jiang Kekai started the belt button of Line 2, felt that the belt was abnormal, and immediately pressed to stop the belt of Line 2. At 8:33, Ma Rong walked from the tail wheel of the belt to the counterweight wheel, heard shouting, and saw Wu Xiukun stuck between the belt and the counterweight wheel. Ma Rong immediately called for rescue through the walkie-talkie.
(2) Emergency disposal. After hearing the call from the walkie-talkie, Yu Jian, the on-site management personnel
of Ruiding Company, and others immediately rushed to the accident site (at the belt counterweight wheel of Line 2) and organized personnel to start rescue at 8:35. During the process, they dialed 120 and reported to the relevant leaders of the company. By taking measures such as cutting the belt and steel frame, the injured person Wu Xiukun was taken out from between the counterweight wheel and the belt at 9:32 and rescued. Around 9:40, 120 ambulances arrived at the scene and carried out rescue, and then sent the wounded to Panzhihua Iron and Steel Hospital for treatment. At 10:15, they died after rescue.
(3) The conclusion of emergency response assessment.
1. After the accident, Yu Jian, the site manager of Ruiding Company, immediately rushed to the accident site (the belt counterweight wheel of Line 2) after hearing the call from the walkie-talkie. In the process of rushing to the accident site, he dialed 120 and reported to the relevant leaders of the company, and organized personnel to start rescue at 8:35. Measures such as cutting belts and steel frames were taken to remove the injured Wu Xiukun from between the counterweight wheel and the belt at 9:32 and rescue him. At 9:40, 120 ambulances arrived at the scene for rescue and sent the wounded to Panzhihua Iron and Steel Hospital for treatment.
2. Sichuan Ruiding Commodity Concrete Co., Ltd. has formulated the Emergency Management System for Production Safety, which clarifies the company's emergency management organization and stipulates the preparation, revision, training and drilling of emergency rescue plans for production safety accidents. Construction Branch of Panzhihua Gangcheng Group Kuntai Engineering Technology Co., Ltd. and Panzhihua Jiuli Construction Labor Co., Ltd. have not established and improved the safety production responsibility system and safety production rules and regulations.
3. Sichuan Ruiding Commodity Concrete Co., Ltd., Panzhihua Steel City Group Kuntai Engineering Technology Co., Ltd. and Panzhihua Jiuli Construction Labor Co., Ltd. have not compiled contingency plan in accordance with the requirements of the Guidelines for the Compilation of contingency plan for Production Safety Accidents in Production and Business Units (GB/T 29639-2020). Sichuan Ruiding Commodity Concrete Co., Ltd. has worked out the on-site disposal plan for the injury accident of belt conveyor and equipment rotating parts, and has carried out relevant emergency drills. Construction Branch of Panzhihua Gangcheng Group Kuntai Engineering Technology Co., Ltd. and Panzhihua Jiuli Construction Labor Co., Ltd. have no relevant emergency drill records.
4. After the accident, the District Committee and the district government attached great importance to it. After receiving the accident report, the District Emergency Management Bureau immediately started the emergency procedure and rushed to the scene with the District Economic and Information Bureau, Nongnongping Street and the Eastern Branch of the Municipal Public Security Bureau to guide the emergency rescue work and start the preliminary investigation. In accordance with the Regulations on the Reporting and Investigation of Production Safety Accidents (Decree No.493 of the State Council), and in accordance with the Approval of the Office of the People's Government of the Eastern District of Panzhihua City on the Request for Authorization to Investigate and Deal with General Production Safety Accidents of District-owned Production and Business Units (Letter No.9 of the Eastern District Government of Panzhihua City), the People's Government of the Eastern District has been authorized. On April 1, 2023, an accident investigation team led by the District Emergency Management Bureau, with the participation of the District Economic and Information Bureau, the District Federation of Trade Unions, the Eastern Branch of Panzhihua Public Security Bureau and the Nongnongping Street Office, was set up, and the District Procuratorate and the District Discipline Commission were invited to participate. The accident disposal process and response time limit are in line with the regulations.
(4) The handling of the aftermath.
After the accident, Jiuli Labor Service Company actively cooperated in dealing with the aftermath and reached a compensation agreement with the family members of the deceased.
