2022, the Emergency Management Bureau of Pukou District of Nanjing City partially collapsed the foundation pit on the north side of the second phase project of Nanjing Bank Science and Education Innovation
Park, located in Nanjing High-tech Zone (Pukou Park). The accident caused two deaths, two minor injuries and one minor injury, resulting in a total direct economic loss of 989.
On December 25, 2021, Nanjing Pukou District Government announced the second phase project of Nanjing Bank Science and Education Innovation Park. 6. The excavation area of the foundation pit is large, the north foundation pit is deep, and the space-time effect is obvious. The actual bearing capacity of the foundation pit supporting system can not meet the safety requirements of the foundation pit, and the collapse is caused by the failure of the pile-anchor system at the accident site.
The indirect reason is that the geotechnical investigation is not comprehensive and accurate enough. The geological survey report issued by the geological survey unit fails to accurately reflect the occurrence of the rock stratum, the shape and slope of the rock surface; the buried depth of the foundation, the occurrence of the rock stratum and the weak structural layer are not verified; the conclusion of the survey report is inconsistent with the verification survey of the site collapse area and the actual situation.
The survey unit has not made necessary modifications and improvements to the design scheme, and has not met the dynamic design requirements. Therefore, Nanjing Sujie Geotechnical Survey and Design Co., Ltd. was found to be mainly responsible for the accident.
The reason is not that the quality of the survey report of Sujie Company is not up to standard. Instead, without the consent of Sujie Company, the relevant units adopted the survey report of Sujie Company beyond the scope, that is, the survey report of the first underground floor (5 meters) of the construction project was applied to the design and construction of the third underground floor (17 meters).
Because the location of the accident is not within the scope of the investigation of Sujie Company, Sujie Company has nothing to do with the accident. Article 28 of the Regulations on the Administration of Survey and Design of
Construction Projects stipulates: "Construction units, construction units and supervision units shall not modify the survey and design documents of construction projects.". The modification unit shall bear corresponding responsibilities for the modified survey and design documents. The relevant units and personnel shall be investigated for their legal liability for the illegal use of the survey report.
According to the requirements of relevant engineering investigation regulations and specifications, the buried depth of the basement foundation of the first floor of the project is only 5-7 meters underground, covering a thick soil layer. The engineering investigation operation does not need to find out the attitude of the rock stratum, nor does it involve the analysis of the impact of the attitude of the rock stratum on the foundation pit support system.
As the overall engineering design scheme of the project has actually been changed, which is a major matter, the law stipulates that the survey unit must be notified to re-survey. The relevant units neither notified the survey unit nor organized a re-survey, but directly adopted the survey report before the design change without authorization.

The accident happened at
about 4:48 p.m. on June 15, 2021. The sudden local collapse of the support in the middle of the BC section of the north side of the foundation pit of the second phase project of the Bank of Nanjing Science and Education Innovation Park (about 77 m in length and 17 m in height)
is the direct cause
of the complex engineering geological conditions of the site, and the rock surface inclines to the pit with a large inclination, which has a negative impact on the stability of the free surface of the foundation pit. The excavation area of the foundation pit is large, and the foundation pit on the north side is deep, so the influence of space-time effect is obvious. The actual bearing capacity of the foundation pit supporting system can not meet the safety requirements of the foundation pit, and the collapse is caused by the failure of the pile-anchor system at the accident site.
Indirect reasons:
(1) The geotechnical investigation is not comprehensive and accurate.
(2) The dynamic design method is not adopted.
(3) The construction of information law has not been implemented.
(4) Lack of awareness of project risk management and control.
(5) Confusion of project management and lack of quality control and safety management.

Wang Moumou, the project manager of Nanjing Sujie Geotechnical Survey and Design Co., Ltd. and the auditor of the geotechnical report, has insufficient knowledge of the complex geological conditions and fails to accurately reflect the occurrence of the rock strata, the shape and slope of the rock surface in the survey report. In view of the geotechnical conditions exposed by the earthwork excavation, the influence of the attitude of the rock stratum on the foundation pit support system was not analyzed, and he was responsible for the accident.
Handling suggestion: The district judicial organ shall investigate the criminal responsibility according to law.
2. Zhang Moumou, the director of Geotechnical Design Institute of Jiangsu Jianyuan Geotechnical Engineering Survey Co., Ltd. and the actual person in charge of the project, has insufficient knowledge of the complex geological conditions, has not adopted the dynamic design method, and has not calculated the reliability of slope support according to the geotechnical conditions exposed by earthwork excavation; Improper analysis and judgment of potential risks such as continuous alarm of monitoring data bear an important responsibility for the accident.
Handling suggestion: The district judicial organ shall investigate the criminal responsibility according to law.
3. Dong Mou-mou, the project manager of the general contractor of the construction, did not urge the relevant participating units to construct strictly in accordance with the design documents, relevant specifications and earthwork excavation construction schemes, did not have a strong awareness of the risk control of the project, did not have enough knowledge of the complex geological conditions, and did not adopt the information method in the construction process. Failure to timely reflect the geological conditions such as rock mass occurrence to the design, survey, supervision and construction parties bears an important responsibility for the accident.
Handling suggestion: The district emergency management department shall impose administrative penalties on the illegal and irregular acts according to law.
4. Wang Moumou, the project director of Nanjing Suning Engineering Consulting Co., Ltd., did not conscientiously perform his supervisory duties, and did not effectively manage the construction unit's failure to comply with the design documents and relevant specifications, and the over-excavation of the construction period; It did not organize all parties involved in the construction to analyze, judge and properly handle the potential risks such as continuous alarm of monitoring data, cracks in the crown beam and drainage ditch of BC section on the north side of the foundation pit, and was responsible for the accident.
Handling suggestion: The district emergency management department shall impose administrative penalties on the illegal and irregular acts according to law.
5. Huang Moumou, the project manager of Nanjing Financial City Construction and Development Co., Ltd., did not conscientiously perform the responsibilities of the project leader of the construction unit in accordance with the regulations, did not verify the inconsistency between the base elevation in the support design and construction scheme and the survey report, did not follow up and supervise the implementation of the recommendations of the survey report, and was responsible for the accident.
Suggestion: The urban and rural construction department of the district shall impose administrative penalties on its violations of laws and regulations according to law.
6. Zhu Moumou, the former deputy general manager of Nanjing Sujie Geotechnical Survey and Design Co., Ltd., did not verify the excavation depth of the foundation pit and the elevation of the base, and did not analyze the impact of the rock formation on the foundation pit support system, so he was responsible for the accident.
Handling suggestion: The enterprise shall handle the case in accordance with the internal rules and regulations, and report the handling situation to the district people's government.
7. Tang Moumou, the technical director of the project of the general contractor, did not urge the relevant participating units to construct strictly in accordance with the design documents and relevant specifications, did not adopt the information method for construction, and did not timely reflect to the design, survey, supervision and construction parties the adverse effects of geological conditions such as rock mass occurrence on the supporting structure of the deep foundation pit, and was responsible for the accident.
Handling suggestion: The enterprise shall handle the case in accordance with the internal rules and regulations, and report the handling situation to the district people's government.
8. Zhang Mou-mou, the construction worker of the general construction contractor, failed to discover and stop the construction of the relevant participating units in strict accordance with the design documents and relevant specifications in time, and was responsible for the accident.
