2022, a major collapse accident occurred at the construction site of Hengqin Taiwanese Business Headquarters Building (main project) in Hengqin Guangdong-Macao Deep Cooperation Zone (hereinafter referred to as the "Cooperation Zone"), resulting in three deaths, five injuries (two of them minor injuries) and a direct economic loss of 4.71 million yuan.

19:00 to 21:08 on the evening of March 24th, after the application of Huang Chengyue, the safety officer of Fujian High Scale Company, and the approval of Li Yukang, the director of the Engineering Department of the Shanghai Baoye Company's Project Department. The tower crane team of CNNC Huaxing Dafeng Company (tower crane driver Chen Fang, floor tower crane signal worker Li Mingrong, and ground tower crane signal worker Song Jianqun) used 3 # tower crane to cooperate with Fujian High Scale Company to lift the finished steel bars processed by 4 cranes to the steel bar turnover platform on the 23rd floor of the core tube. 8:30, the steel bar workers in the ground steel bar processing area completed the hooking of the steel bar weighing about 2100 kg. Zhang Minghua, the signal worker of the ground tower crane, informed Yue Hongli, the driver of the tower crane, to lift the hook through the walkie-talkie. After the rebar is lifted, the tower crane arm rotates counterclockwise and swings to the top of the tower core tube. After Peng Xiuqiong, the tower crane signalman on the 23rd floor, confirms the position of the object to be lifted, he informs Yue Hongli to hook slowly through the walkie-talkie. When the steel bars were lifted to the vicinity of the steel bar turnover platform, three workers came forward to meet them.

Two workers working on the 20th floor were hit by a falling collapse (Figure 7). The above 8 workers were buried on the 20th floor by the falling collapse (Figure 8).

After emergency rescue at the scene, the accident eventually resulted in 3 deaths and 5 injuries (3 people were sent to hospital for treatment; 2 people were slightly injured and got out of trouble at the scene).

After winning the bid, due to the delay of the project and the agreement with the Taiwanese Business Headquarters Building Company, there were only 5 on-site supervisors (1 chief supervision engineer, 2 professional supervision engineers, 1 supervisor and 1 data clerk). The mechanical and electrical professional supervisor who applies for construction shall not work in the project department, and shall sign the relevant materials in the project department from time to time. The on-site mechanical and electrical supervision work shall be replaced by others. At the time of

the accident, the project had five professional projects, including civil engineering, steel structure, fire protection, electromechanical and curtain wall, with 267 people working at the same time, and the staffing of the supervision organization could not meet the actual needs of the supervision task. Huang Zhangxiang,

the chief supervision engineer, Feng Yuehua, the civil engineering professional supervision engineer, and Wang Bin, the site supervisor, are not clear about the stipulation that the post of the rigger needs to be certified and can not be confused with the separate operation of the signal worker.

Song Shengming and Cheng Qi, the two project leaders registered with the qualification of first-class registered constructors, did not actually fulfill their responsibilities for the safety production management of the projects involved, and there were problems of unequal powers and responsibilities. Except for Cheng Qi and Chen Bo, other personnel of the Project Department have no written notice of appointment.

(2) The project management is chaotic, and the safety management work is not implemented in place. Sixteen special operation operators in the project were suspected of having false certificates. (3) The safety supervision measures for the I-beam of the core tube reinforcement turnover platform are not in place. (4) The safety management of tower crane construction is out of control and pipe leakage. (5) Steel structure installation construction is organized without special construction scheme. (6) The main management personnel falsify the record registration and use the labor dispatch personnel in violation of regulations. Among the existing 29 employees of the

Project Department, 19 employees are not affiliated with Shanghai Baoye Company (including relevant branches) in terms of social insurance, and 16 of them belong to labor dispatch personnel.

Accident liability determination and handling suggestions are

transferred to judicial organs for handling (5 persons)

1. It is suggested that they be transferred to judicial organs for investigation of their criminal responsibility according to law, and that they shall not be the principal person in charge of any construction unit or the person in charge of the project within five years from the date of completion of the execution of the penalty.

(2) Li Yukang, male, a member of the masses, is the director of the Engineering Department and the construction supervisor of the Shanghai Baoye Company's Incident Project Department, responsible for safe production and coordination at the construction site. It is suggested that they be transferred to judicial organs for investigation of their criminal responsibility according to law.

It is suggested that the case be transferred to the judicial organ for investigation of criminal responsibility according to law; it is suggested that the Urban Planning and Construction Bureau of the Cooperation Zone shall, in accordance with the Regulations on the Administration of Safe Production of Construction Projects and other relevant provisions, make corresponding treatment to the relevant registered qualification certificates held by him according to law.

(5) Huang Chengyue, male, a member of the public, is the site safety officer of the civil engineering of the incident project of Fujian High Scale Company. It is suggested that they be transferred to judicial organs for investigation of their criminal responsibility according to law.

The above five persons suggested that after the judicial organs have dealt with the matter according to law, the enterprises concerned or their superior authorities should give relevant administrative sanctions in a timely manner in accordance with their administrative authority.

Thirteen people were placed on file for investigation! It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 95 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities.

(2) Liu Bingjin, male, native of Taiwan, China, Engineering Director of Taiwan Business Headquarters Building Company. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities.

(4) Jin Feng, male, member of the Communist Party of China, deputy general manager of the Shanghai Baoye Guangzhou Branch, and responsible person in charge of the western Guangdong region. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities. For the violations of discipline involved in accident prevention, it is suggested that they be handed over to the relevant Disciplinary Commission for investigation and handling according to law.

(5) Cheng Qi, male, a member of the Communist Party of China, is the person in charge of the Shanghai Baoye Company's construction application project. It is suggested that the Commercial Service Bureau of the Cooperation Zone shall file a case for investigation in accordance with Article 96 of the Production Safety Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal liabilities; it is suggested that the Urban Planning and Construction Bureau of the Cooperation Zone shall, in accordance with the Regulations on the Administration of Production Safety of Construction Projects and other relevant provisions, deal with its relevant registered qualification certificates in accordance with the law. For the violations of discipline involved in accident prevention, it is suggested that they be handed over to the relevant Disciplinary Commission for investigation and handling according to law.

(6) Qiu Xiang, male, member of the public, director of the incident project security department of Shanghai Baoye Company. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities. It is suggested that the duty violations involved in accident prevention should be handed over to the relevant supervisory committee for investigation and treatment according to law.

(7) Su Wencan, male, member of the public, director of the incident project technology department of the Shanghai Baoye Company. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities. It is suggested that the duty violations involved in accident prevention should be handed over to the relevant supervisory committee for investigation and treatment according to law.

(8) Qi Xiangjie, Male, Mass, Material Supervisor of Material Department of Shanghai Baoye Company's Incident Project. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities.

(9) Chen Zhengrong, male, a member of the public, is the safety officer of the incident project of Shanghai Baoye Company. It is suggested that the Commercial Service Bureau of the Cooperation Zone should file a case for investigation in accordance with Article 96 of the Safety Production Law of the People's Republic of China and other laws and regulations, and investigate its relevant legal responsibilities.

(14) Peng Xiuqiong, female, tower crane signal worker of the incident project of CNNC Huaxing Dafeng Company. It is suggested that the Urban Planning and Construction Bureau of the Cooperative Zone should transfer the relevant competent authorities to deal with the relevant qualifications of the Qualification Certificate for Special Construction Operations held by the Cooperative Zone in accordance with the Notice on the Issuance of the Regulations on the Management of Special Construction Operators (Jianzhi [2008] 75), and the relevant qualifications of the Qualification Certificate for Special Construction Operations held by the Cooperative Zone should be handled by the China Nuclear Huaxing Dafeng Company in accordance with the internal management regulations. The processing results shall be copied to the Urban Planning and Construction Bureau of the Cooperation Zone. If the

relevant administrative functional departments find that the units or personnel responsible for the accident have committed other illegal acts in violation of the provisions of laws, regulations and rules in the course of investigation, they shall be given administrative penalties and dealt with in accordance with the relevant provisions. If the person responsible for the accident is suspected of committing a crime, it is suggested that the judicial organ investigate the criminal responsibility in accordance with the relevant provisions of the Criminal Law.

It is suggested that the personnel

who have been punished by Party discipline and government affairs should hand over the relevant clues to the investigation and investigation team of the Discipline Inspection and Supervision Committee of the Cooperation Zone for the problems and related materials found in the investigation of the accident concerning the performance of duties and responsibilities of the public officials of the relevant departments and units. The opinions on the punishment of Party discipline and government affairs of the relevant personnel shall be put forward by the Discipline Inspection and Supervision Committee of the Cooperation Zone.
