Organize and carry out special inspection of the city's testing institutions and ready-mixed concrete enterprises in the second half of the year. The inspection is hereby notified as follows:

I. Basic Information

This inspection covers 5 testing institutions and 23 ready-mixed concrete enterprises within the jurisdiction.

(1) For testing institutions. The inspection shall focus on whether the original records and test reports meet the requirements; whether the testing personnel, instruments and equipment, and working environment (place) of the testing institution meet the qualification requirements; whether the quality assurance system is established and implemented as required, whether the test report review system is implemented, and whether the test contract is filed. From the inspection situation, most of the testing institutions can comply with the relevant regulations, standards and norms to carry out the testing work, for the problems found in the inspection, a total of 4 orders for correction were issued, and 30 hidden dangers of quality and safety were found.

(2) For ready-mixed concrete enterprises. It mainly inspects the quality control of ready-mixed concrete production, the quality control of raw materials, the quality inspection of ready-mixed concrete, the laboratory conditions and the availability of personnel, and samples the quality of raw materials. II. Existing problems

(I) Testing institutions

1. Account information: individual testing institutions did not establish a summary account of unqualified reports; the unqualified account did not record the process information of reporting to customers; some testing entrustment agreements were altered without explanation; The operation time of each four times is not filled in the operation record sheet of core drilling report; there is no daily use record sheet for the instruments in the asphalt mixture test room; there is no relevant acceptance record for the sand and gravel used for additive testing.

2. Instruments and equipment: the use records of instruments and equipment of individual testing institutions were not signed by the users; the waterproof material room was not equipped with standard curing equipment for waterproof coatings, which could not meet the curing requirements; the chemical laboratory was not equipped with spraying devices; the rubber pad was placed between the base and the base of the vibrating table for cement testing, and the mortar was not laid. 3.In terms of

testing process: the rebound method of some testing institutions is used to test the compressive strength of concrete in batches, but there is no testing scheme; the expression of "compaction coefficient (%)" in the test record form of sand base compactness is incorrect, and the sample is only kept for 3 days after the test, but not for 7 days; the sample has been leaked in the concrete impermeability test, and the record is not made in time.

4. Inspection and monitoring: there is only one person in the video recording of on-site inspection of reinforcement protective layer of individual inspection institutions, which does not meet the requirements, and the video recording of plate components does not cover the whole process; the video monitoring of rebound components does not capture the carbonization value; the panoramic monitoring of impermeability test can not capture, and the coverage of the monitoring system is insufficient; Rules and regulations related to video monitoring and daily use records are not established; there is no four-way placard in the video recording of steel bar protective layer thickness detection, and there is no witness to confirm the component parts.

5. Filing of testing contract: the testing contract for Phase I of Plot P21 of Nan'anmei's smart home project was signed on January 3, 2022, and the contract filing date was April 4, 2022, which was not filed in time; Huachuang Hejingyuan had no testing contract and was not filed with the quality and safety station.

(II) Ready-mixed concrete enterprises

1. Laboratory management: the testers of individual enterprises are not familiar with the standards and specifications of test operation; they are unable to provide corresponding treatment measures when the concrete strength is abnormal or fails to meet the specified requirements; there is no double lock management for chemicals; The reserved samples of cement, fly ash and other samples are less than 6 kg; the specific surface area of cement, room temperature and humidity do not meet the requirements; some volumetric cylinders and cement mortar test molds are not self-calibrated; the water reducing rate of admixture does not include the water content of admixture, and the calculation is not accurate to 0.1%.

2. Mix proportion: there is no post responsibility and work flow for concrete mix proportion adjustment in some enterprises, or the post authorization for mix proportion adjustment is not clear; the design and calculation process of mix proportion is not clear; the test method of setting time is wrong; the pavement flexural mix proportion is not designed according to the requirements of road specifications, and the mix proportion has not been verified for more than half a year.

3. Green mixing station: the newly added aggregate silos of some concrete enterprises are not closed; the intercepting ditch of the aggregate silo is blocked; the entrance and exit of the aggregate silo and the solid waste point of the mud press have serious dust accumulation; the wall spraying system is damaged and can not be used.

4. Safety production: some concrete enterprises did not weigh the ready-mixed concrete mixer trucks; the safety production training records were incomplete; the safety warning signs in the production plant area were insufficient; there were no relevant safety production publicity slogans.

5. Spot check of raw materials: a group of fly ash from individual enterprises was sampled, and the strength activity index of this group of fly ash did not meet the technical requirements of Class F II fly ash for mixing concrete and mortar.

3. Disposal Decision

(1) Each inspection institution shall compare the problems found in the inspection with the Notice on Further Standardizing the Qualification Examination and Approval of Construction Quality Inspection Institutions (Min Jian Xu [2020] No.4). The Measures for the Administration of Quality Inspection of Construction Projects (Decree No.141 of the Ministry of Construction) and other relevant provisions require strict implementation of rectification, drawing inferences from one instance, and strengthening self-examination and self-correction.

(II) All ready-mixed concrete enterprises shall carefully rectify and feedback the problems found in the inspection in accordance with the requirements of the Quality Management Standards for Ready-mixed Commercial Concrete in Fujian Province (2018 Revision) and other relevant provisions; our Bureau shall deal with the problems in accordance with the Regulations on Dynamic Assessment Management of Ready-mixed Concrete Enterprises and other relevant provisions, and give dynamic scores to the enterprises. Quanzhou Jinzhou Concrete Co., Ltd., which failed to pass the spot check of raw materials, was ordered to rectify in accordance with the requirements of Fujian Provincial Quality Management Standard for Ready-mixed Commercial Concrete (Revised Edition 2018). Fujian Jindoushan Building Material Development Co., Ltd. and Fujian Quanzhou Datang Concrete Co., Ltd., which scored more points, were included in the daily differential management.

4. Relevant requirements

(1) All testing institutions should effectively improve the testing capabilities of laboratories and on-site testing projects, and do a good job in dangerous goods, sample management, contract filing, equipment verification and other matters according to relevant requirements.

(2) All ready-mixed concrete enterprises shall make timely rectification in accordance with the requirements of the Notice of Order for Correction, conduct in-depth self-examination and self-correction, strictly implement the laws, regulations and standards for the production management of ready-mixed concrete enterprises, improve the internal management system and quality control measures, ensure the effective operation of the management system, and ensure that the management system is effective. The quality of raw materials, the quality of the production process, and the quality of the transportation process should be effectively controlled; the training of highly skilled personnel in key positions should be increased, and the cultivation of professional quality should be strengthened.

(3) All ready-mixed concrete enterprises shall strengthen the management of green mixing stations and implement low-carbon energy saving, low noise, low emission, recycling of waste water and waste residue.
