After preliminary investigation, this is a liability accident, because the operators in the installation of cyclone fixtures and gas cutting operations, the falling cutting slag ignited combustible materials and caused a fire, which exposed the following prominent problems existing in the enterprises involved, relevant localities and departments: First, the enterprises involved take risks and operate brutally; The second is that the fire prevention and extinguishing measures on site are defective; the third is that the operation is carried out in advance regardless of safety; the fourth is that the implementation of safety management responsibilities is not in place; and the fifth is that the lessons learned from the accident are not serious.

First, the enterprises involved took risks and operated brutally. BOT cooperation mode (investment, construction and operation of the project are entrusted to the same enterprise) is adopted for the desulfurization and denitrification project in which the accident occurred. Party A and Party B are respectively Baotou Steel Rare Earth Steel Plate Co., Ltd. (Hereinafter referred to as Baotou Steel Plate) and Shandong Guoshun Construction Group Co., Ltd. (Hereinafter referred to as Shandong Guoshun). When Shandong Guoshun formulated the overall operation plan for inspection and maintenance, the overall plan was not considered enough, and when it was found that there were major deficiencies in the operation plan and the installation of flue cyclones needed to be increased, the temporary supplementary plan formulated by Shandong Guoshun did not fully consider that there were a large number of flammable materials on the inner wall and bottom of the flue gas pipeline of the desulfurization and denitrification system, and only put forward the equipment. Necessary measures such as collecting cutting slag with funnel anti-splash device, polishing and removing inflammables around the hot work point, and physical quarantine of inflammables below the hot work point were not included, and the actual implementation was not in place. Especially in the absence of effective quarantine with the other two working points in the same pipeline system, the fire was eventually caused, resulting in the death of adjacent workers.

Second, there are defects in fire prevention and extinguishing measures on site. The level of this hot work was originally "Level I hot work", but because it was carried out at night, it should be upgraded to "special hot work" level, and the corresponding level of safety and fire prevention measures should be formulated and implemented, but Shandong Guoshun still managed according to "Level I hot work" during on-site operation. In the process of operation, the implementation of safety measures was insufficient, only two fire extinguishers were equipped on site, and the fire water connection was not made according to the requirements of the operation plan, which could not meet the needs of fire fighting after the fire broke out.

Third, regardless of safety, rush to work. The project was originally planned to stop production for maintenance for 15 days. During the operation, Shandong Guoshun found that there were major deficiencies in the original maintenance plan, and temporarily increased the installation of flue cyclones requiring more than 5 days, but did not extend the total construction period, nor did it increase the number of operators. The inspection and maintenance of desulfurization system includes a large number of dangerous operations such as limited space, hot work and high altitude, and multiple dangerous factors such as falling from high altitude, electric shock, poisoning and asphyxia are intertwined. However, Shandong Guoshun only cares about interests and ignores safety. In order to complete all inspection and maintenance operations within the established time limit, it arranges hot work and other operations at the same time without full demonstration, which eventually leads to accidents.

Fourth, the implementation of safety management responsibility is not in place. The safety management of the subordinate enterprises of Baotou Steel Group and Baotou Steel Union is not in place, and the chain of responsibility for safety management is not compacted layer by layer. Although it is clear in the agreement signed by Baotou Steel Union Plate and Shandong Guoshun that Party a should supervise the safety precautions in the course of Party B's operation, in the actual performance of the contract, Baotou Steel Union Plate is out of control of the inspection and maintenanc e operation content and safety precautions of Shandong Guoshun, and does not examine and assess the safety risks of the inspection and maintenance operation plan. It has not performed the unified coordination and management of its safety production work, nor has it carried out regular safety inspection on the project site. Shandong Guoshun is known as a professional air pollution control and environmental protection enterprise, but of the 121 on-site construction personnel of the desulfurization and denitrification inspection and maintenance project, only 9 are formal employees, the rest are all labor dispatch workers, and have not carried out systematic safety training, and have not fulfilled the safety training responsibilities of labor dispatch personnel.

Fifth, it is not serious to draw lessons from accidents. The National Video Conference on Safety Production Law Enforcement and Industry and Trade Safety Supervision held on March 4, as well as the recent circular issued by the Office of the Security Committee of the State Council on the three accidents, clearly require relevant enterprises and regulatory departments to draw lessons from the "2.6" accident of Maanshan Iron and Steel Co., Ltd. and the "2.15" accident of Shanghai Waigaoqiao Power Plant. Carefully investigate and identify the safety problems of environmental protection facilities, outsourcing of process links, inspection and maintenance, take other people's accidents as their own lessons, pay close attention to the investigation, and strictly prevent minor matters from causing major disasters. However, only 10 days after the end of the meeting, an accident occurred during the inspection and maintenance of the environmental protection facilities of Baotou Steel's steel plate, indicating that Baotou Steel Group and Shandong Guoshun did not seriously learn from the accident lessons of other enterprises, and did not investigate and treat the hidden dangers of environmental protection facilities such as dust collectors, desulfurization and denitrification as required; the work deployment and supervision and inspection work of the territorial supervision department were not in place; Relevant parties have not strictly checked the design, manufacture, construction, acceptance and operation of environmental protection facilities, and regard other people's accidents as stories.

In order to draw lessons profoundly and resolutely reverse the current trend of frequent accidents in iron and steel enterprises and environmental protection facilities, the following work requirements are put forward once again:

First, draw lessons, draw inferences about other cases from one instance, and fully implement safety control measures. Relevant enterprises should draw lessons from Maanshan Iron and Steel Co., Ltd., Waigaoqiao Power Plant and the accident, compare the problems exposed by the accident with their own reality, thoroughly investigate and rectify the hidden dangers of the problems, and truly prevent risks, eliminate hidden dangers and curb accidents. In order to improve the safety operation ability of key post personnel, especially those related to environmental protection facilities, organize safety warning education and training, plug loopholes from the source, we must not "forget the pain after healing the scar" and repeat the same mistakes repeatedly. It is necessary to fully identify all kinds of safety risks in the process of operation, put forward targeted safety measures, and incorporate preventive and remedial measures in advance, including not only "airbags" but also "emergency brakes". It is necessary to strengthen on-site safety monitoring of high-risk operations, strictly enforce the examination and approval system for operations, implement measures to deal with dangerous factors, equip emergency response equipment, and strictly prevent measures from being hung on the wall, left on paper, and stopped on the mouth.

Second, further implement the responsibilities of all parties in detail. In accordance with the provisions of Article 49 of the Safety Production Law, enterprises should strengthen the unified coordination and management of the safety production work of contracting and leasing units, strictly fulfill the safety production responsibilities of all parties stipulated in the agreement, strictly prevent the emergence of "vacuum zones" in the safety management boundary, and strictly prohibit replacing safety supervision with trust and safety management with past performance. Enterprise headquarters and superior companies should improve and perfect the safety management mechanism, compact the safety production responsibility chain of sub-companies (branches) and dispatched agencies at all levels layer by layer, and strictly prevent the weakening and relaxation of safety management layer by layer. Emergency management departments at all levels should give full play to the role of comprehensive supervision departments, work with relevant supervision departments, effectively use closure and rectification, joint punishment for dishonesty, interview and exposure, firmly grasp the "key minority" of the principal responsible persons of enterprises, and firmly promote the implementation of the main responsibility of enterprises for safe production. In accordance with the requirements of the "three necessities", all relevant departments should assume their respective responsibilities, regard safety production as an important part of industry management, implement supervision and management in accordance with the law and regulations, and earnestly assume the responsibility of industry management.

3. Strengthen the safety management of inspection and maintenance operations. Enterprises should strengthen the safety management of inspection and maintenance and outsourcing operations, comprehensively identify the main safety risks in each link, and scientifically formulate inspection and maintenance plans. The examination and approval procedures shall be strictly carried out for the temporarily added operation contents, and the safety impact of the temporarily added operation on the existing inspection and maintenance operation and whether the conditions for parallel operation are met shall be fully demonstrated. It is strictly prohibited to add work without safety management. It is necessary to sort out the number of dispatched workers and their work, strictly enforce the regulations on the scope and proportion of employment, strictly implement the safety education and training system, and earnestly ensure that they are trained before taking up their posts. Special operators must hold certificates according to the regulations. Emergency management departments at all levels should strengthen supervision, inspection and guidance services for enterprise inspection and maintenance operations, so as to strengthen supervision and strictly enforce the law to promote the implementation of safety production management responsibilities of all parties.

4. Highlighting the safety investigation of environmental protection facilities. Each enterprise shall comprehensively arrange the environmental protection facilities such as dust remover, desulfurization and denitrification, and comprehensively sort out the safety management of projects with different cooperation modes such as BOT, full trusteeship operation and outsourcing operation. First, we should highlight the full coverage, carefully sort out the various business contracts that are still in the implementation period, find out the basic information of the project type, scale and relevant units, and establish a list of accounts; second, we should highlight the re-investigation, conduct a comprehensive physical examination of the project in the implementation period, clarify the safety production management responsibilities of all parties according to law, and strictly prohibit contracting for management and illegal subcontracting; Thirdly, we should highlight zero tolerance, and resolutely withdraw from the field and never tolerate the outstanding problems found, especially the inadequate on-site safety management of high-risk operations, the non-implementation of the operation approval system, and the formality of on-site monitoring. Relevant departments should strengthen the key supervision of enterprises with intensive environmental protection facilities such as dust collectors, desulfurization and denitrification, carefully sort out the situation of environmental protection facilities projects in their respective administrative areas under the BOT mode and the full trusteeship operation mode, and find out the bottom number, especially the operation of environmental protection facilities in their respective administrative areas by Shandong Guoshun and other similar operators one by one, and carry out law enforcement. Those who fail to fulfill their responsibilities and commit illegal acts should be dealt with and held accountable in accordance with the law and regulations.
