In accordance with the Reporting, Investigation and Handling of Work Safety Accidents The relevant laws and regulations of the Regulations (Decree No.493 of the State Council) stipulate that on April 10, entrusted by the Banner Government and led by the Banner Emergency Response Bureau, the Banner Public Security Bureau, the People's Social Bureau, the General Trade Union and the Discipline Inspection Commission formed an accident investigation team to investigate and deal with the accident. In accordance with the principles of "four not letting go" and "scientific and rigorous, in accordance with the law, seeking truth from facts and paying attention to practical results", the
accident investigation team, through on-site investigation, investigation and evidence collection, identified the course, causes, casualties and direct economic losses of the accident, identified the nature and responsibility of the accident, and put forward the responsible units. In view of the causes of the accident and the prominent problems exposed, the accident prevention measures and suggestions are put forward, and the relevant information is reported as follows:
I. The accident unit

(II) Inner Mongolia Xutai Hoisting Co., Ltd.
is located at Room 502, Unit 3, Building 14, Wenhua Shangjing Phase II, Bazhen, Azuo Banner. Its legal representative is Du Dongze. Its business scope includes heavy cargo lifting, hoisting and handling; crane leasing, etc. Liu Rongguo, the truck crane driver assigned to Inner Mongolia Songta Cement Co., Ltd., holds the Skilled Post Construction Operation Certificate (Intermediate) issued by Hebei Construction Machinery Association Housing and Urban-Rural Construction Industry, Certificate No.: 2019JX02QCSQZD02993, issued on March 23, 2020 (the certificate is not recognized).
(III) Contract Signing
On March 1, 2020, Inner Mongolia Songta Cement Co., Ltd. (Party A) and Inner Mongolia Xutai Hoisting Co., Ltd. (Party B) signed the Crane Hoisting Service Contract. Inner Mongolia Xutai Hoisting Co., Ltd. shall timely arrange vehicles to arrive at the site for construction according to the needs of crane hoisting business for production line operation of Inner Mongolia Songta Cement Co., Ltd., and the validity period is from March 1, 2020 to December 31, 2020.
2. Accident process and rescue situation
On April 8, 2020, Inner Mongolia Songta Cement Co., Ltd. carried out a comprehensive overhaul of the plant and workshop, and the preheater C4 platform (accident platform) kiln tail tower in the clinker workshop was hoisting castables (refractory materials) and preparing for the replacement of castables. A total of 45 operators were arranged on that day, of which the team led by Shi Licheng, the team leader, consisted of 12 people, responsible for the hoisting and handling of castables on C2 and C4 platforms. According to the original hoisting plan, the material was lifted by the kiln tail winch. Due to the slow speed and low hoisting efficiency, the crane hired by the company had free time, so Liu Jie, the director of the clinker workshop, arranged the crane and Shi Licheng to carry out the hoisting operation. At 8:30, Shi Licheng found Liu Rongguo, the driver of the crane, and asked him to hoist refractory materials. Shi Licheng directed the crane to lift the castable to the designated position through the walkie-talkie on the accident platform. Workers Li Nanga, Ma Zhanzhen and Yang Xueshan carried the castable lifted by the crane to the preheater. Workers Liu Huiqin, Feng Xiuping, Li Weili and Zhao Yanling carried out auxiliary handling under the preheater. At 11:05, when the castables were hoisted and transported on the C4 platform of the preheater in the clinker workshop of Inner Mongolia Songta Cement Co., Ltd., the platform extended to the north platform (31.12 hours above the ground), Shi Licheng died after being rescued by the Central Hospital.
3. Casualties and direct economic losses

IV. Causes of the accident and the nature
of the accident (1) Causes

is that when the head of the clinker workshop of Inner Mongolia Songta Cement Co., Ltd.
(2) The employees of Inner Mongolia Songta Cement Co., Ltd. did not stack refractory materials in the designated "refractory brick stacking area" according to the design requirements, but directed the crane to stack refractory materials on the C4 extension platform. The platform belongs to the personnel channel during routine inspection and maintenance, and belongs to illegal command.
(3) The site safety management of Inner Mongolia Songta Cement Co., Ltd. is not in place, and the illegal operation and command behavior are not corrected in time. The castables for hoisting are continuously stacked on the side of C4 extension platform, reaching more than 3 tons. There is a rush to catch up with the construction period and progress. Finally, due to overloading, the castables for hoisting are stacked on the side of C4 extension platform. As a result, the steel structure on one side of the C4 extension platform fell off and the platform collapsed.
(II) Nature
of the accident According to the analysis of the causes of the above accident, this is a general production safety liability accident of falling from a high place caused by the collapse of the extension platform due to illegal operation, illegal command and overloading during the on-site hoisting operation.
5. Identification of accident liability and suggestions
for dealing with those responsible for the accident (1) Inner Mongolia Songta Cement Co., Ltd., the risk control of safety production site is not in place, the implementation of safety precautions is not in place, and the employees violate the rules of operation, command and operation in the process of hoisting castables. According to the first paragraph of Article 109 of the Safety Production Law, it is suggested that the Emergency Management Bureau of Azuo Banner should impose an economic penalty of 250000 yuan on Inner Mongolia Songta Cement Co., Ltd.
(2) Dou Yanbin, as the general manager of Inner Mongolia Songta Cement Co., Ltd., failed to supervise and inspect the work safety of the unit and eliminate the hidden dangers of production safety accidents in time, resulting in accidents, according to the provisions of Article 92, paragraph 1, of the Safety Production Law. It is suggested that the Emergency Management Bureau of Azuo Banner impose a penalty of 30% of Dou Yanbin's annual income in the previous year.
(3) Shi Licheng, the head of the clinker workshop of Songta Cement Co., Ltd. in Inner Mongolia, did not wear the safety belt as required when he was working at high altitude on duty, which was an illegal operation. He directed the crane driver to lift the castables and stack them on the extension platform of the operation. Because of the excessive quantity, the castables piled up collapsed the platform on which they stood. Because he died in the accident, he was exempted from accountability.
(4) Liu Jie, the director of the clinker workshop of Songta Cement Co., Ltd. in Inner Mongolia, as a full-time (part-time) safety administrator on site, failed to strictly perform his duties of safety management, failed to remind and supervise the workers working at high places to wear labor protection articles (safety belts), in violation of the provisions of Article 22 (6) of the Safety Production Law. According to Article 45 of the Measures for Punishment of Illegal Acts in Production Safety, it is suggested that the Emergency Management Bureau of Azuo Banner impose a fine of RMB 9000 on Liu Jie.
At the same time, Azuo Banner Emergency Management Bureau should draw lessons from the accident, strengthen the supervision and management of enterprise safety production, strictly implement the main responsibility of enterprise safety production, and inspect the Party Group of Azuo Banner Emergency Management Bureau by the captain of Azuo Banner Safety Production Comprehensive Law Enforcement Brigade and the director and deputy director of Azuo Banner Emergency Management Bureau.
6. Accident prevention and rectification measures
The accident exposed that Inner Mongolia Songta Cement Co., Ltd. could not operate in strict accordance with the "Safety Work Ticket at Heights", did not have a strong sense of safety in production, did not stack refractory materials in the designated refractory brick stacking area in accordance with the design requirements, and had problems such as catching up with the construction period and speeding up the progress. In order to draw lessons from the accident and prevent similar accidents, the following rectification suggestions are put forward:
(I) Inner Mongolia Songta Cement Co., Ltd. should draw profound lessons from the accident, further strengthen the safety production site management of the unit, strictly implement the responsibility for safety production, strengthen team building, and regularly carry out "three-level" safety education and training. Highlight the training of post operation rules, strictly examine and approve operation tickets (certificates), enhance the safety awareness, self-protection awareness and safety operation skills of employees, make good training evaluation records, stop and correct the "three violations" in the process of operation, especially increase the investigation and punishment of the "three violations".
(2) Inner Mongolia Songta Cement Co., Ltd. should conduct a comprehensive safety inspection of on-site facilities and equipment, highlight the safety management of inspection and maintenance of facilities and equipment, scientifically and rationally arrange the operation process, especially the comprehensive analysis of the carrying capacity of each working platform, strengthen research and judgment, and enhance the risk identification ability of employees. We will resolutely put an end to the hidden dangers of safety and accidents caused by catching up with the construction period and progress, and prevent similar accidents.
(III) Inner Mongolia Songta Cement Co., Ltd. shall further screen the construction qualification and operation qualification of equipment, facility lessors and personnel, establish accounts, carry out disclosure operations, rectify the existing problems as soon as possible, and form a rectification report to be submitted to relevant units.
